摘要

目的

探索胎龄28周以上孕妇非意外因素导致早产的原因，并对其危险因素进行分析。

方法

选取2024年1月1日至2024年12月31日在北部战区总医院妇产科定期产检且分娩的孕妇5 167例，根据其是否早产分为早产组（466例）和足月组（4 701例）。通过问卷调查的方法收集孕妇的基本资料，采用Logistic多因素回归分析早产的危险因素。

结果

早产组产妇高龄、流产史、妊娠期糖尿病（GDM）、妊娠期高血压（HDCP）、妊娠期贫血、妊娠合并肥胖、孕期阴道炎、胎膜早破、前置胎盘占比均高于足月组，差异均有统计学意义(

0.05)。多因素Logistic回归分析显示高龄、HDCP、妊娠期贫血、妊娠合并肥胖、胎膜早破、前置胎盘是孕妇早产的危险因素(

结论

孕妇早产与多种因素相关，其中高龄、HDCP、妊娠期贫血、妊娠合并肥胖、胎膜早破、前置胎盘是其危险因素，临床上应密切关注孕妇状态，及时排除危险因素，处置突发情况。

Abstract

Objective

To explore the causes of preterm birth in pregnant women with gestational age more than 28 weeks due to non-accidental factors， and analyze the risk factors.

Methods

A total of 5 167 pregnant women who underwent regular prenatal check-ups and delivered in the Department of Obstetrics and Gynecology of the General Hospital of the Northern Theater Command from January 1， 2024 to December 31， 2024 were selected and divided into the preterm birth group （466 cases） and the full-term group （4 701 cases） according to whether they were preterm. The basic information of pregnant women was collected through questionnaire surveys， and Logistic multivariate regression analysis was used to analyze the risk factors of preterm birth.

Results

The proportions of advanced maternal age， history of abortion，gestational diabetes mellitus （GDM），hypertensive disorder complicating pregnancy （HDCP），gestational anemia，pregnancy complicated with obesity， vaginitis during pregnancy， premature rupture of membranes， and placenta previa in the preterm birth group were significantly higher than those in the full-term group， with statistically significant differences （

0.05）. Multivariate Logistic regression analysis showed that advanced maternal age， HDCP， anemia during pregnancy， pregnancy complicated with obesity， premature rupture of membranes， and placenta previa were the risk factors for preterm birth in pregnant women （

0.05）.

Conclusion

Preterm birth in pregnant women is associated with multiple factors， among which advanced maternal age，HDCP，anemia during pregnancy，pregnancy complicated with obesity， premature rupture of membranes， and placenta previa are risk factors. Clinically，the condition of pregnant women should be closely monitored， risk factors should be promptly eliminated， and emergencies should be handled.
