目的
2
Objective
2
To compare the accuracy of X‑ray, ultrasonography and MRI in evaluating the lesion extent of invasive breast cancer with extensive intraductal component (EIC), and to provide imaging evidence for precise preoperative assessment.
Methods
2
A total of 185 patients with EIC-positive invasive breast cancer who underwent surgery at PLA Rocket Force Characteristic Medical Center from January 2020 to March 2026 were retrospectively enrolled. All patients completed preoperative X‑ray, ultrasonography and MRI examinations. The maximum diameter of the lesion measured by postoperative pathology was used as the gold standard to evaluate the measurement accuracy and consistency of the three imaging modalities, and stratified analysis was performed according to calcification status and MRI lesion classification.
Results
2
MRI measurements showed good correlation with the pathological diagnosis (Spearman correlation coefficient
r
=0.738,
P
<
0.001), while X‑ray(
r
=0.657,
P
<
0.001) and ultrasonography(
r
=0.511,
P
<
0.001) showed moderate correlation. In the non-calcified lesion subgroup, MRI achieved the optimal consistency with pathological diagnosis (ICC=0.793,
P
<
0.001), reaching an excellent level; in the calcified lesion subgroup,X‑ray yielded the best consistency (ICC=0.700,
P
<
0.001), reaching a good level. MRI maintained good consistency with pathological diagnosis across al
l three lesion subtypes, with the highest consistency for mass with non-mass enhancement lesions (ICC=0.685,
P
<
0.001). The proportion of patients with absolute measurement error
<
5 mm was the highest in MRI (57.3%), while the proportion of patients with measurement error
>
10 mm was the highest in ultrasonography(29.7%).
Conclusion
2
All three imaging modalities can be applied to preoperative lesion extent assessment of invasive breast cancer with EIC. MRI has the best overall accuracy,X‑ray has greater advantages in evaluating calcified lesions, and ultrasonography can serve as an initial screening modality and be combined with other examinations to achieve comprehensive assessment.
