摘要

目的

明确艾司氯胺酮预处理在儿童腹腔镜下疝囊高位结扎术中的应用效果。

方法

选择在全身麻醉下行腹腔镜下疝囊高位结扎术的患儿共70例，按照随机数字表法分为对照组和观察组，每组35例。对照组患儿实施常规全身麻醉，观察组患儿在全身麻醉诱导前15 min，静脉注射艾司氯胺酮0.5 mg/kg。对比2组患儿术前焦虑评分、呼吸力学指标、肺损伤状况、麻醉恢复情况及苏醒期不良反应。

结果

观察组患儿麻醉前10 min和麻醉前即刻焦虑评分显著低于对照组（

0.05）。观察组患儿建立气腹30 min和手术结束时的气道峰压和平均气道压均显著低于对照组（

0.05）。观察组患儿手术结束时的白细胞介素-6（IL-6）和肿瘤坏死因子α（TNF-α）水平均显著低于对照组（

0.05）。2组患儿自主呼吸恢复时间、意识清醒时间、气管拔管时间和麻醉后恢复室（PACU）停留时间比较，差异均无统计学意义（

0.05）。观察组患儿麻醉苏醒期不良反应发生率显著低于对照组（

0.05）。

结论

艾司氯胺酮预处理可以显著缓解腹腔镜下疝囊高位结扎患儿的术前分离焦虑情绪，减轻肺损伤，降低气道压，有利于肺保护和术后麻醉恢复，且不良反应少。

Abstract

Objective

To determine the application effect of esketamine pretreatment in laparoscopic high ligation of hernia sac in children.

Methods

A total of 70 children who received laparoscopic high ligation of hernia sac under general anesthesia were selected and divided into the control group and the observation group according to the random number table method， with 35 cases in each group. The control group received conventional general anesthesia， while the observation group was intravenously injected with 0.5 mg/kg of esketamine 15 minutes before the induction of general anesthesia. The preoperative anxiety score， respiratory mechanics indexes， lung injury status， anesthesia recovery， and adverse reactions during the awakening period were compared between the two groups.

Results

The anxiety scores of the observation group 10 minutes before anesthesia and immediately before anesthesia were significantly lower than those of the control group （

0.05）. The peak airway pressure and mean airway pressure of the observation group 30 minutes after establishing pneumoperitoneum and at the end of the operation were significantly lower than those of the control group （

0.05）. The levels of interleukin-6 （IL-6） and tumor necrosis factor-α （TNF-α） at the end of the operation in the observation group were significantly lower than those in the control group （

0.05）. There was no statistically significant difference in the time of spontaneous breathing recovery， time to regain consciousness， tracheal extubation time， or post-anesthesia care unit （PACU） stay time between the two groups（

0.05）. The incidence of adverse reactions during the anesthetic awakening period in the observation group was significantly lower than that in the control group （

0.05）.

Conclusion

Esketamine pretreatment can significantly alleviate preoperative separation anxiety for children undergoing laparoscopic high ligation of hernia sac， reduce lung injury， and decrease airway pressure， which is beneficial to lung protection and postoperative anesthetic recovery， and has few adverse reactions.
