摘要

目的

探讨增强现实（augmented reality，AR）技术-B超融合导航在复杂动静脉内瘘精准穿刺中的有效性与安全性。

方法

纳入2023年1月至2023年12月于宁波大学附属第一医院接受规律血液透析治疗的75例复杂动静脉内瘘患者，按随机数字表法分为对照组（采用传统方式进行盲穿）、B超干预组和AR联合B超干预组，每组25例。比较3组基线资料、穿刺效果、满意度以及穿刺疼痛评分。采用双重差分模型分析3组满意度及穿刺疼痛评分。采用Cox回归分析不同干预措施对患者并发症发生风险的影响。

结果

AR联合B超干预组操作时间、调针次数明显低于其他2组，而一次穿刺成功率明显高于其他2组（

0.05）。随访3个月、6个月，AR联合B超干预组满意度评分明显高于其他2组（

0.05），穿刺疼痛评分明显低于其他2组（

0.05）。双重差分结果显示，与B超干预组及对照组相比，AR联合B超干预组可明显提升患者满意度评分并降低穿刺疼痛评分，且该干预效应在随访6个月时进一步增强，改善幅度大于对照组和B超干预组。随访6个月，AR联合B超干预组总并发症发生率明显低于对照组（

0.05）。Cox回归分析结果显示，以对照组为参照，校正相关混杂因素后，AR联合B超干预组仍可明显降低患者并发症发生风险（

0.05）。

结论

AR技术辅助B超引导穿刺可明显提升复杂动静脉内瘘的穿刺成功率、降低穿刺相关并发症风险，改善患者体验与远期血管预后，是一种安全、有效的精准化穿刺干预策略。

Abstract

Objective

To investigate the effectiveness of augmented reality（AR）-assisted ultrasound in precise puncture of complex arteriovenous fistulas.

Methods

A total of 75 Patients with complex arteriovenous fistulas who received regular hemodialysis treatment in The First Affiliated Hospital of Ningbo University from January to December 2023 were enrolled and divided into control group（using the traditional method for blind puncture），ultrasound intervention group，and AR-assisted ultrasound intervention group using a random

number table，with 25 patients in each group. The three groups were compared in terms of baseline data，puncture outcomes，satisfaction score，and puncture pain score. A difference-in-differences model was used to analyze the satisfaction score and puncture pain score of the three groups. A

Cox

regression analysis was used to assess the impact of different interventions on the risk of complications in patients.

Results

Compared with the control group and the ultrasound intervention group，the AR-assisted ultrasound intervention group had a significantly shorter operation time，a significantly lower number of needle adjustments，and a significantly higher first-attempt success rate of puncture（

0.05）. At 3 and 6 months of follow-up，the AR-assisted ultrasound intervention group had a significantly higher satisfaction score and a significantly lower puncture pain score than the other two groups（

0.05）. The difference-in-differences analysis showed that compared with the ultrasound intervention group and the control group，AR-assisted ultrasound intervention significantly improved satisfaction score and reduced puncture pain score，and this intervention effect was further enhanced at 6 months of follow-up，with the improvement significantly greater than that in the control group and the ultrasound intervention group. At 6 months of follow-up，the AR-assisted ultrasound intervention group had a significantly lower overall incidence rate of complications than the control group（

0.05）. The

Cox

regression analysis showed that with the control group as a reference and after adjustment for relevant confounding factors，AR-assisted ultrasound intervention still significantly reduced the risk of complications（

0.05）.

Conclusion

AR-assisted ultrasound-guided puncture can significantly improve the success rate of puncture for complex arteriovenous fistulas，reduce the risk of puncture-related complications，and improve patient experience and long-term vascular prognosis，and therefore，it is a safe and effective strategy for precise puncture intervention.
