摘要

目的

探讨反复直立训练对1例帕金森患者合并严重直立性低血压（orthostatic hypotension，OH）的疗效。

方法

训练前行直立倾斜试验（head-up tilt test，HUTT），给予每日1次反复直立训练8次后，复查HUTT。继续给予1次/d反复直立训练8次后复查HUTT。比较3次HUTT结果，判断疗效。

结果

第1次HUTT复查结果显示直立耐受时间由2.5 min延长至15 min。第2次HUTT结果显示直立耐受时间延长至35 min。最后，患者的OH严重程度分级由3级变为2级。

结论

反复直立训练对严重OH有快速和显著的治疗效果。

Abstract

Objective

To investigate the therapeutic effect of repetitive upright training in the treatment of a patient with Parkinson’s disease and severe orthostatic hypotension（OH）.

Methods

The head-up tilt test（HUTT） was performed before training and after 8 sessions of daily repetitive upright training. Another 8 sessions of daily repetitive upright training were performed，and then HUTT was performed again. The results of the three HUTT assessments were compared to evaluate therapeutic effect.

Results

The first HUTT reexamination showed that the upright tolerance time was prolonged from 2.5 minutes to 15 minutes. The second HUTT reexamination showed that the upright tolerance time was prolonged to 35 minutes. Finally，the severity of OH in this patient was reduced from grade 3 to grade 2.

Conclusion

Repetitive upright training has a rapid and significant therapeutic effect on severe OH.
