摘要

目的

分析Tei指数联合实时三维超声心动图（real-time three-dimensional echocardiography，RT-3DE）评估冠心病（coronary artery heart disease，CHD）左心功能的可行性，并探讨其与肌酸激酶同工酶（creatine kinase isoenzyme-MB，CK-MB）、白细胞介素-23（interleukin-23，IL-23）的相关性。

方法

回顾性选取2023年1月至2025年6月于安徽医科大学附属亳州医院经冠状动脉造影确诊的100例CHD患者作为观察组，并以同期100名健康体检者作为对照组。比较2组的一般资料、超声指标以及血清指标。采用Pearson相关性分析血清指标与超声参数之间的相关性。

结果

观察组的收缩压、左心室舒张末期内径（left ventricular end diastolic dimension，LVEDD）、左心室收缩末期内径（left ven

tricular end-systolic diameter，LVESD）、左心房前后径（left atrial diameter，LAD）、室间隔厚度（interventricular septum thickness，IVST）、左室后壁厚度（left ventricular posterior wall thickness，LVPWT）、E/e值、等容收缩时间（isovolumic contraction time，ICT）、等容舒张时间（isovolumic relaxation time，IRT）、Tei指数、左室舒张末期容积（left ventricular end diastolic volum，LVEDV）、左室收缩末期容积（left ventricular end-systolic volume，LVESV）以及CK-MB、IL-23水平均高于对照组，而左心室射血分数（left ventricular ejection fraction，LVEF）、e值、e/a值、射血时间（ejection time，ET）、左室每搏输出量（left ventricular stroke volume，LVSV）、左室整体纵向峰值应变（global longitudinal strain，GLS）绝对值低于对照组（

0.05）。相关性分析显示，LVEDD、LVESD、LAD、IVST、LVPWT、E/e、ICT、IRT、Tei指数、LVEDV、LVESV、GLS与CK-MB、IL-23均呈正相关，而LVEF、e、e/a、ET、LVSV与CK-MB、IL-23均呈负相关（

0.05）。

结论

Tei指数联合RT-3DE可有效评估冠心病患者的左心功能，且CK-MB、IL-23与各超声指标存在相关性，以上指标可用于临床辅助评估。

Abstract

Objective

To investigate the feasibility of Tei index combined with real-time three-dimensional echocardiography （RT-3DE） in assessing left ventricular function in coronary heart disease（CHD），as well as its correlation with creatine kinase isoenzyme-MB（CK-MB） and interleukin-23（IL-23）.

Methods

A total of 100 patients with CHD who were diagnosed by coronary angiography in Bozhou Hospital Affiliated to Anhui Medical University from January 2023 to June 2025 were retrospectively enrolled as observation group，and 100 healthy individuals who underwent physical examination during the same period of time were enrolled as control group. The two groups were compared in terms of general data，ultrasound parameters，and serological parameters，and the Pearson correlation analysis was used to investigate the correlation between serological and ultrasound parameters.

Results

Compared with the control group，the observation group had significantly higher levels of systolic blood pressure，left ventricular end-diastolic diameter （LVEDD），left

ventricular end-systolic diameter（LVESD），left atrial diameter（LAD），interventricular septal thickness（IVST），left ventricular posterior wall thickness（LVPWT），E/e value，isovolumic contraction time（ICT），isovolumic relaxation time（IRT），Tei index，left ventricular end-diastolic volume（LVEDV），left ventricular end-systolic volume（LVESV），CK-MB，and IL-23，as well as significantly lower levels of left ventricular ejection fraction（LVEF），e value，e/a value，ejection time（ET），left ventricular stroke volume （LVSV），and the absolute value of left ventricular global longitudinal strain（GLS）（

0.05）. The correlation analysis showed that LVEDD，LVESD，LAD，IVST，LVPWT，E/e value，ICT，IRT，Tei index，LVEDV，LVESV，and GLS were positively correlated with CK-MB and IL-23，while LVEF，e value，e/a value，ET，and LVSV were negatively correlated with CK-MB and IL-23（

0.05）.

Conclusion

Tei index combined with RT-3DE can evaluate left ventricular function in patients with CHD，and CK-MB and IL-23 have a certain correlation with ultrasound parameters. These indicators can be used for clinical auxiliary evaluation.
