Objective To evaluate the value of dynamic contrast-enhanced MRI (DCE-MRI) in the quantitative assessment of microvascular permeability of brain glioma. Methods Clinical data of 38 patients with pathologically confirmed brain gliomas admitted in Southwest Hospital in the year of 2014 were collected in this study. They all received 3.0 T MRI scanning including DCE-MRI to obtain the highest value of volume transfer constant (K
trans
) and volume of extravascular extracellular space per unit
volume of tissue (Ve). The differences in Ktrans and Ve between the different grades and the correlations between K
trans
Ve and glioma grade were statistically analyzed. Results The values of K
trans
and Ve were significant lower in grade Ⅱ glioma (0.028±0.011 min
-1
0.081±0.044) than grade Ⅲ (0.105±0.025 min
-1
,0.456±0.079) and grade Ⅳ (0.138±0.053 min
-1
,0.512±0.112)(P0.01)
but there were no differences between grade Ⅲ and grade Ⅳ glioma (P>0.05). Positive correlation was found in Ktrans and Ve with glioma grade
and between K
trans
and Ve. Conclusion DCE-MRI can be used to quantitatively assess microvascular permeability and to determine pre-operative grading of glioma.
