Objective To compare and evaluate clinical applications of Cook criterion (2004)
international diabetes federation (IDF) criterion (2007) and Chinese criterion (2012) for metabolic syndrome (MetS) in children and adolescents. Methods A total of 1 493 children and adolescents aged in 10-17 years old in Chongqing were enrolled in the study. Physical examination and blood biochemical examination were taken. The prevalence of MetS
and the diagnostic concordance
sensitivity and specificity of three criteria for MetS were compared. Results The prevalence rates of MetS were 2.68%
5.02%
and 7.03%
respectively
based on the IDF criterion
Chinese criterion
and Cook criterion
and were shown as the obesity groupthe overweight groupthe normal weight group (P0.05). The anomaly detection rate of 2 or more components was shown as the obesity groupthe overweight group the normal weight group (P0.05)
and the three-component abnormal combination was central obesity+high triglyceride+lipid metabolic abnormality. The Kappa indices between the Chinese criterion and IDF criterion
between the Chinese criterion and Cook criterion
and between the IDF criterion and Cook criterion were 0.667
0.729
and 0.505
respectively. Taking one diagnostic criteria as the “gold criteria”
the sensitivity of the combination of the other 2 criteria was increased. Conclusion Chinese criterion is more effective than the IDF criterion in the detection of adolescents with high risk of cardiovascular diseases. It is helpful to use the 3 diagnostic criteria in parallel for MetS diagnosis
so as to improve the diagnostic sensitivity. The Chinese criterion and Cook criterion have high consistency
and can achieve high sensitivity and specificity when used in parallel.
