摘要

目的

针对我国药学门诊面临的发展不均衡、服务规范差异大、患者认知不足及医保覆盖有限等问题，制定《药学门诊规范化实施指南（2026年版）》。

方法

由中国医院协会药事专业委员会、中国药学会循证药学专业委员会和海峡两岸医药卫生交流协会医院药学专业委员会牵头，组织国内19名医院药学专家，通过系统梳理国家政策、文献研究，总结当前实践经验，经充分讨论后，对指南内容达成共识。结果与

结论

本指南涵盖药学门诊的定义与内涵、开设条件、实施管理、岗位胜任力以及实践研究5个部分。其中，定义与内涵包括药学门诊的3种形式（独立、医药联合、互联网药学门诊）及分类服务模式（专科、专药、专病药学门诊）；开设条件包括制度建设（药学服务管理制度、质量控制与评估机制）、人员要求（专业资格、继续教育与专业培训等）、服务对象、服务场所与设施设备；实施管理包括服务流程、干预措施、表单记录、患者教育与随访、人文关怀、风险管理与质量控制；岗位胜任力包括药学门诊出诊药师岗位胜任力和教学方法建议；实践研究则鼓励在药学门诊环境下开展高质量药学实践。本指南为我国药学门诊的规范化实施提供了开设、管理、教学与研究等方面的有益指导，填补了该领域指南的空白，推动了药学门诊的高质量发展。

Abstract

OBJECTIVE

To formulate

Guidelines for the standardized implementation of pharmacist-managed clinics

（

2026 edition

） in response to the challenges faced by such clinics in China， including uneven development， large discrepancies in service specifications， insufficient patient awareness， and limited medical insurance coverage.

METHODS

Led by the Pharmaceutical Affairs Professional Committee of the Chinese Hospital Association， the Evidence-based Pharmacy Professional Committee of the Chinese Pharmaceutical Association， and the Hospital Pharmacy Professional Committee of the Cross-strait Medical and Health Exchange Association， a total of 19 domestic hospital pharmacy experts were organized. Through a systematic review of national policies and literature research， current practical experience was summarized. Consensus on the contents of the guidelines was reached after in-depth discussions.

RESULTS &

CONCLUSIONS

The guidelines covered five sections： definition and connotation of pharmacist-managed clinics， establishment requirements， implementation and management， post competency， and practical research. Firstly， the definition and connotation included three operational forms of pharmacist-managed clinics （independent mode， physician-pharmacist joint mode， and online pharmacist-managed clinic mode） and classified service modes （specialty-specific， drug-specific， and disease-specific pharmacist-managed clinics）. The establishment requirements were further refined， covering system construction （pharmaceutical service management system， quality control and assessment mechanism）， personnel qualifications （professional credentials， continuing education and professional training， etc）， service recipients， as well as service venues and facilities. Subsequently， the implementation and management of pharmacist-managed clinics were proposed， involving service procedures， intervention measures， documentation and records， patient education and follow-up， humanistic care， as well as risk management and quality control. Finally， post competency encompassed the competency requirements for pharmacists providing services in pharmacist-managed clinics， as well as the suggestions on teaching methods； practical research encouraged the conduct of high-quality pharmaceutical practice in the setting of pharmacist-managed clinics. The guidelines provide valuable guidance for the standardized implementation of pharmacist-managed clinics in China in terms of establishment， management， teaching， and research， fill the guideline gap in this field， and can promote the high-quality development of pharmacist-managed clinics.
