Development of a post competency-based curriculum index system for the INSIGHT model in urinary system specialty training
To develop a post competency-oriented curriculum index system for general practitioners in urological diseases and to provide an evidence-based framework for the institution-coordinated specialty-integrated general practitioner holistic training (INSIGHT) model.
Methods
2
A mixed-methods approach combining literature review
outpatient diagnostic data analysis
and semi-structured interviews was used to collect qualitative data
identify core training content
and construct a preliminary curriculum index system. Two rounds of Delphi consultation were conducted with experts experienced in clinical practice
teaching administration
or primary healthcare management in general practice
nephrology and urology. Quantitative evaluation data were collected. Statistical analyses were performed using SPSS 27.0. Descriptive statistics
including frequency
percentage
mean
and standard deviation were used to analyze quantitative data
while experts' open opinions were used to revise and improve indicators. The analytic hierarchy process was employed to assign values to the importance of indicators at all levels
and weighting coefficients of indicators were calculated using the geometric mean and square root methods.
Results
2
Twenty-four experts participated in the consultation. The questionnaire recovery rates in the first and second rounds were 100.00% and 83.33%
respectively
with authority coefficients of 0.848 and 0.870. All indicators received mean importance scores
>
3.50 and coefficient of variation
<
0.25. In the second round
Kendall's coordination coefficients for the three levels of indicators were 0.454
0.214
and 0.214 (
P
<
0.05)
indicating a certain degree of coordination in expert opinions. A curriculum index system comprising 6 first-level indicators
16 second-level indicators
and 46 third-level indicators was established.
Conclusions
2
The curriculum index system developed in this study provides a structured and evidence-based framework for the standardized implementation of the INSIGHT model
and offers a referential paradigm for the development and optimization of other organ system-specific training curriculums.
