摘要

目的

探讨“AI导师-人类导师”协同教学模式在住院医师规范化培训（住培）教学病例系统中的应用效果，为智能医学教育转型提供实践参考。

方法

基于武汉协和医院教学病例系统，构建人机协同教学模式。采用非随机、前后对照的观察性设计，以2025年7月AI智能体部署为节点，对比部署前（2025年4至6月，

=1 738）与部署后（2025年8至10月，

=235）学员的问卷反馈情况。问卷信效度良好（CVI=0.92，Cronbach’s

=0.76），采用卡方检验与效应量（Cramer’s

）进行分析。

结果

模式实施后，学员认为临床思维训练有帮助的认可率从79.69%提升至88.51%（

²=9.62，

=0.002，

=0.07）；认为填报耗时

60 min的学员比例从62.66%提升至67.23%（

²=16.22，

0.001，

=0.09）；认为病例设计合理的学员比例从78.60%提升至89.79%（

²=15.36，

=0.09）。所有指标的改善均具有统计学意义（

0.05），效应量虽弱，但方向一致。

结论

“AI导师-人类导师”协同教学模式可有效优化教学流程、提升互动质量及临床思维训练效果。其成功实施需以“人机能力矩阵”明确分工，并配套开展组织、评价与伦理等“生产关系”调整。建议在推广过程中注重跨部门协同、师资培训与数据治理，以推动医学教育的内涵式发展。

Abstract

Objective

To explore the application effectiveness of the artificial intelligence (AI) mentor-human mentor collaborative teaching model in a teaching case system for standardized residency training

and to provide a practical reference for the transformation toward intelligent medical education.

Methods

A human-AI collaborative teaching model was constructed based on the teaching case system at Union Hospital. A non-randomized

before-and-after observational design was adopted

and an AI agent was deployed in July 2025. Trainee questionnaire feedback before deployment (April-June 2025

=1 738) was compared with that after deployment (August-October 2025

=235). The questionnaire demonstrated good reliability and validity (content validity index=0.92

Following model implementation

the recognition rate of trainees for the helpfulness of clinical thinking training increased from 79.69% to 88.51% (

=0.07). The proportion of trainees who reported spending less than 60 minutes on case entry increased from 62.66% to 67.23% (

the directions of changes were consistent.

Conclusions

The AI mentor-human mentor collaborative teaching model can effectively optimize the teaching process

enhance interaction quality

and improve the effectiveness of clinical thinking training. Its successful implementation requires a clear division of labor based on a human-AI capability matrix and supporting adjustments in organizational

evaluative

and ethical "production relations". It is recommended to focus on cross-departmental collaboration

faculty training

and data governance during promotion to promote the connotative development of medical education.
