摘要

目的

探讨TSPV四步教学（理论教学、模拟操作训练、临床实践训练、手术录像点评）联合反思提示卡在普外科住院医师规范化培训中的应用效果。

方法

选取2022年2月至2024年10月于川北医学院附属医院普外科规培的150名住院医师为研究对象，按入科时间分为对照组（

=75，传统带教）和研究组（

=75，TSPV四步教学联合反思提示卡）。通过理论考试、操作技能考核、临床思维评估量表及教学满意度问卷进行综合评价。采用SPSS 22.0进行

检验和Mann-Whitney

检验。

结果

研究组在理论考核的案例分析题得分[（24.80±2.85） vs. （22.12±3.48），

=0.004

]

和操作技能总分[（87.68±5.52） vs. （81.48±7.24），

=0.001

]

均高于对照组。临床思维总分研究组为（84.16±6.05）分，高于对照组的（73.12±9.88）分，且在风险预判、动态决策、循证决策等子维度优势明显（均

0.001）。满意度调查显示，研究组在教学方式、反馈机制、临床思维提升等维度的评分更高（均

0.001）。

结论

TSPV四步教学联合反思提示卡能有效促进普外科规培医师理论向实践的转化，提升操作技能和高阶临床思维能力，教学效果显著，值得推广。

Abstract

Objective

To explore the application effectiveness of TSPV four-step teaching (theoretical teaching

simulated operation training

clinical practice training

and surgical video review) combined with reflection prompt cards in the standardized training of general surgery resident physicians.

Methods

A total of 150 resident physicians who received standardized training in the Department of General Surgery at the Affiliated Hospital of North Sichuan Medical College from February 2022 to October 2024 were selected as the research subjects. They were divided into a control group (

=75

traditional mentorship-based teaching) and a research group (

=75

TSPV four-step teaching combined with reflection prompt cards) according to their enrollment time. Comprehensive evaluation was conducted through theoretical exams

operational skill assessments

clinical thinking assessment scales

and teaching satisfaction questionnaires. SPSS 22.0 was used for

Results

The research group scored higher than the control group in the case analysis questions of theoretical assessment [(24.80±2.85) vs. (22.12±3.48)

The TSPV four-step teaching combined with reflection prompt cards can effectively promote the transformation of theory into practice for general surgery training physicians

improve operational skills and advanced clinical thinking abilities

and achieve significant teaching effectiveness

which is worth promoting.
