Characteristics and research prospects of cerebral metabolism in patients with traumatic brain injury
摘要
Abstract
Cerebral metabolism failure is one of the most important pathophysiological changes after traumatic brain injury (TBI). The energy metabolism disorder following TBI is associated with failure to maintain normal ionic homeostasis
excitotoxic releases of glutamate and other excitatory neurotransmitters
free radicals damage
and mitochondrial dysfunction. There are several methods to measure cerebral metabolism
including jugular bulb catheter
cerebral microdialysis (CMD)
positron emission tomography (PET)
and magnetic resonance spectroscopy (MRS). It is of great importance to monitor cerebral metabolism in the TBI patients in order to evaluate the severity of brain injury
predict the risk for development of secondary cerebra injury
guide systemic glucose management
assess the ability of nerve repair
and so on. Monitoring cerebral metabolism has been an important part of multimodal monitoring in severe neurological patients.
