Journal of Third Military Medical University, 2017, 39(3): 276-280. DOI: 10.16016/j.1000-5404.201609009.
Objective
2
To detect the changes of platelet function in the patients with aneurysmal subarachnoid hemorrhage (aSAH) by thromboelastography (TEG)
and investigate the relationship of the results with rebleeding.
Methods
2
A retrospective study was carried out on 259 aSAH patients and 39 patients with unruptured intracranial aneurysms who admitted in the First Affiliated Hospital from August 2014 to August 2015. They all underwent TEG on admission. The relationship of TEG parameters with the results of Hunt-Hess grading system and Fisher grading scale were analyzed. The aSAH patients were further divided into rebleeding and non-rebleeding subgroups. Clinical data
platelet level and TEG parameters were compared between the 2 subgroups.
Results
2
The inhibitory rate of adenosine diphosphate (ADP) was significantly higher in the aSAH patients than the UIA patients (
U
=1 505
P
<
0.01). And the inhibitory rate was positively correlated with Hunt-Hess grade (
r
=0.387
P
<
0.01) as well as Fisher grade (
r
=0.382
P
<
0.01). The aSAH patients with rebleeding had higher inhibitory rates of ADP and arachidonic acid (AA) than those without rebleeding (
P
<
0.05
P
<
0.01).
Conclusion
2
Platelet dysfunction is present after aSAH
and rebleeding is correlated with obvious decline of platelet function. Using TEG for platelet function can be considered as an early warning indicator to assess the risk of rebleeding after aSAH.
