Application of noninvasive beat-to-beat blood pressure monitoring combined with transcranial Doppler in assessment of orthostatic hypotension
Orthostatic hypotension(OH) is a common hemodynamic abnormality in the elderly and in patients with neurological disorders, and it is closely associated with syncope, falls, and cerebral hypoperfusion. Conventional intermittent cuff-based blood pressure measurements are unable to capture the rapid blood pressure fluctuations that occur immediately after postural change, thereby limiting accurate assessment of the dynamic characteristics of OH. Noninvasive beat-to-beat blood pressure monitoring can realize continuous blood pressure measurement with high temporal resolution, while transcranial Doppler(TCD) can provide real-time assessment of cerebral blood flow velocity. The combination of these two techniques can dynamically reflect the relationship between blood pressure decline and cerebral perfusion changes during postural change and thus provide an important basis for the classification of OH,the exploration of its underlying mechanisms, and the assessment of cerebral blood flow regulation. This article reviews the advances in the application of noninvasive beat-to-beat blood pressure monitoring combined with TCD in the research and clinical evaluation of OH and elaborates on the technical advantages, major research findings, and clinical application value of this combined approach,in order to provide a reference for the precise assessment and mechanistic investigation of OH.
