Doses to organs at risk in radiotherapy for cervical cancer based on dose-volume histogram and International Commission on Radiation Units and Measurements Report 89: a comparative study
(International Commission on Radiation Units and Measurements,ICRU)89号报告推荐方法计算的OARs剂量进行比较,分析两者间的差异。
To investigate the differences in the doses to organs at risk(OARs) (bladder and rectum) derived from dose-volume histogram (DVH) during external beam radiotherapy versus those calculated by the method recommended in International Commission on Radiation Units and Measurements(ICRU) Report 89 in patients with cervical cancer undergoing external beam radiotherapy combined with brachytherapy.
Methods
2
A retrospective analysis was performed for 100 patients with cervical cancer who received external beam radiotherapy combined with brachytherapy, with a dose of 45 Gy(25 fractions of 1.8 Gy,once daily,5 times per week) for external beam radiotherapy. The D
2cc
and D
0.1cc
doses of the bladder and the rectum were obtained from the DVH and were converted to equivalent dose in 2 Gy fractions(EQD2),which were then compared with the ICRU dose (i.e.,the EQD2 dose of 43.20 Gy for OARs calculated by the method recommended in ICRU Report 89).
Results
2
The mean EQD2 doses of bladder D
2cc
and D
0.1cc
obtained from the DVH were (45.97±0.59) Gy and (46.67±0.55) Gy,respectively, which were 6.4%(
P
<
0.001) and 8.0%(
P
<
0.001) higher than the ICRU dose;the mean EQD2 doses of rectum D
2cc
and D
0.1cc
obtained from the DVH were (45.46±0.89) Gy and (46.34±0.73) Gy,respectively, which were 5.2%(
P
<
0.001) and 7.3%(
P
<
0.001) higher than the ICRU dose.
Conclusion
2
For patients with cervical cancer undergoing external beam radiotherapy combined with brachytherapy, the doses to OARs calculated using the method recommended in ICRU Report 89 during external beam radiotherapy are significantly lower than the actual doses derived from DVH,thereby underestimating the total doses to OARs,which may increase the incidence rates of radiation-induced cystitis and proctitis. It is recommended to evaluate the doses to OARs based on the actual doses obtained from the DVH during external beam radiotherapy combined with brachytherapy for cervical cancer.
