Value of iron deposition in different brain regions combined with triglyceride-glucose index and neutrophil-lymphocyte ratio in predicting cognitive impairment in elderly patients with type 2 diabetes
To investigate the association of triglyceride-glucose index(TyG) and neutrophil-lymphocyte ratio(NLR) with cognitive impairment in elderly patients with type 2 diabetes,as well as their predictive efficacy and the mediating effect of magnetic susceptibility values of gray matter nuclei between them.
Methods
2
A total of 209 elderly patients with type 2 diabetes were enrolled, and according to Montreal Cognitive Assessment score, they were divided into cognitive impairment group and non-cognitive impairment group. The two groups were compared in terms of clinical data, laboratory data, and quantitative susceptibility mapping parameters. A multivariate logistic regression model was established to investigate the independent risk factors for cognitive impairment;the partial correlation analysis was used to investigate the correlation of TyG and NLR with each cognitive domain in the cognitive impairment group, and a mediating effect analysis was performed;the receiver operating characteristic(ROC) curve was used to assess predictive efficacy.
Results
2
The cognitive impairment group
had significantly higher TyG and NLR than the non-cognitive impairment group(
P
<
0.05). The multivariate logistic regression analysis showed that TyG and NLR were independent risk factors for cognitive impairment. NLR was negatively correlated with visuospatial and executive function(
r
=-0.425,
P
<
0.001),while TyG was negatively correlated with delayed recall(
r
=-0.372,
P
=0.008),attention(
r
=-0.514,
P
<
0.001),and visuospatial/executive function(
r
=-0.367,
P
=0.009). The mediating effect analysis showed that iron deposition in the hippocampus and the globus pallidus exerted a partial mediating effect on the relationship between NLR/TyG and cognitive impairment, while iron deposition in the caudate nucleus only exerted a partial mediating effect on the relationship between TyG and cognitive impairment. In predicting cognitive impairment,NLR or TyG used alone had an area under the ROC curve (AUC) of 0.753 and 0.822,respectively,while the combination of NLR and TyG had an AUC of 0.901.
Conclusion
2
Increases in TyG and NLR are closely associated with cognitive impairment in patients with type 2 diabetes, and the combination of NLR and TyG had a better predictive performance. Iron deposition in the central nervous system exerts a partial mediating effect on the relationship between TyG/NLR and cognitive impairment.
