Applicability and regional adjustment of the six-minute walk distance prediction equation for children in Chongqing,China
To investigate the applicability of the Hong Kong-based six-minute walk distance(6MWD) prediction equation in children in Chongqing,China,to establish locally adapted equations, and to formulate a regional reference standard.
Methods
2
A total of 744 healthy children (358 boys and 386 girls) aged 7-12 years from Chongqing were enrolled, and the standardized six-minute walk test(6MWT) was performed. Growth indicators and 6MWD distribution were systematically compared between the Chongqing cohort and the Hong Kong reference population, and the paired
t
-test,Bland-Altman agreement analysis, and root mean square error (RMSE) were used to quantify the predictive bias of the Hong Kong equation. Based on the local data of Chongqing and with the change in heart rate(ΔHR) and height as independent variables, multiple linear regression was used to establish localized sex-specific 6MWD prediction equations.
Results
2
The Chongqing children generally had lower anthropometric measures and 6MWD measured values in key age groups compared with their Hong Kong peers. The Hong Kong equation significantly overestimated 6MWD in the Chongqing cohort, with a mean bias of +184.67 m and an RMSE of 198.16 m. The newly established localized equations were 6MWD=251.43+2.14×ΔHR+1.55×height for boys and 6MWD=288.61+1.83×ΔHR+1.30×height for girls, with satisfactory model explanation(
R
2
=0.544/0.479). The new equations had a significant improvement in predictive accuracy(RMSE decreased to 50.19 m),a stronger correlation with measured values(
r
=0.719),and the elimination of systematic bias.
Conclusion
2
The Hong Kong-based equation has poor applicability in children in Chongqing. The locally adapted equations for Chongqing developed in this study show a good degree of fitting and accurate prediction, which provides a reliable basis for establishing 6MWD reference values for children in Chongqing.
