摘要

随着人口老龄化与慢性非传染性疾病高发，内分泌代谢疾病负担沉重，已成为医保管理、医保基金支出的重要组成部分。重庆市自2002年推出基本医疗保险门诊特殊疾病保障政策以来，距今已23年，其间相关疾病的循证医学证据逐渐丰富，诊疗理念、治疗技术已经历巨大变革和更新，原有医疗保险政策亟须与时俱进、适应现代医学技术发展，为患者提供更新、更好的医疗保障服务，因此重庆市医疗保障局计划于2025年6月1日起实施新医保政策。为更好地促进政策实施，确保政策顺利运行，重庆市医学会内分泌学分会、糖尿病学分会组织内分泌代谢及公共卫生专家，在重庆市医疗保障局指导下，对内分泌代谢专业相关门诊特殊疾病的诊断标准及报销范围相关新政策进行解读。将最新的内分泌代谢疾病指南与重庆医疗实际相结合，通过多轮专家讨论，结合医保管理经验，就门诊特殊病种报销目录、疾病诊断路径优化、规范化诊疗实施等关键环节进一步细化、具体化，达成专家共识。拟推动建立标准化管理体系，规范诊疗行为，提升医保基金使用效率及疾病管理达标率，遏制不合理医疗支出，确保医保资金精准用于提升患者生存质量，实现慢性病全生命周期管理，助力全面实现“健康中国2030”重庆行动，为全国医保改革提供区域性实践参考。

Abstract

With the aging population and the increasing prevalence of chronic non-communicable diseases， the burden of endocrine and metabolic diseases has become substantial and represents a significant portion of medical insurance management and fund expenditures. Since the introduction of the basic medical insurance outpatient special disease protection policy in Chongqing in 2002， 23 years have passed. During this period， the evidence base for relevant diseases has gradually expanded， and the concepts of diagnosis and treatment， as well as therapeutic technologies， have undergone considerable updates and transformations. The existing medical insurance policy urgently needs to be updated to keep pace with modern medical advancements and to provide patients with more advanced and effective medical security services.Therefore， starting from June 1， 2025， the Medical Insurance Bureau of Chongqing Municipality will implement a new medical insurance policy reform. To ensure the smooth implementation and operation of this policy， the Endocrinology and Diabetes Branches of the Medical Association of Chongqing Municipality have organized experts in endocrine metabolism and public health. Under the guidance of the Medical Insurance Bureau， these experts have interpreted the new policies related to the diagnostic criteria and reimbursement scope for outpatient special diseases in the field of endocrine metabolism. By integrating the latest guidelines for endocrine and metabolic diseases with the actual medical practice in Chongqing， and through multiple rounds of expert discussions that combine clinical experience with medical insurance management insights， a consensus has been reached on key aspects such as the reimbursement directory for outpatient special diseases， optimization of disease diagnostic pathways， and the implementation of standardized diagnosis and treatment protocols. This consensus aims to promote the establishment of a standardized management system.The goal is to enhance the efficiency of medical insurance fund utilization and the rate of disease management achievement， to standardize diagnostic and treatment practices， to curb unreasonable medical expenditures， and to ensure that medical insurance funds are precisely allocated to improve patients' quality of life. This initiative will help achieve full life cycle management of chronic diseases， support the comprehensive implementation of the "Healthy China 2030" action plan in Chongqing， and provide a regional practice reference for the national medical insurance reform.
