摘要

目的

探讨高尔基体蛋白73（golgi protein 73，GP73）在原发性胆汁性胆管炎（primary biliary cholangitis，PBC）诊断中的作用及其与疾病进程和治疗疗效监测的相关性。

方法

收集70例PBC患者、36例非自身免疫性肝病（autoimmune liver disease，AILD）的其他肝病患者（non-AILD）和40例健康体检者（healthy control，HC）血清，酶联免疫吸附试验（enzyme linked immunosorbent assay，ELISA）测定血清中GP73的水平。其中，PBC住院患者在治疗结束前、后分别收集血清样本进行GP73检测。

结果

血清GP73浓度在PBC组、non-AILD组和HC组的分布上，差异有统计学意义（

0.001），受试者工作特征（receiver operating characteristic，ROC）曲线显示GP73在诊断PBC的曲线下面积（the area under the curve，AUC）为0.839。血清GP73水平与天门冬氨酸氨基转移酶（aspartate aminotransferase，AST）（

=0.337，

=0.009）、碱性磷酸酶（alkaline phosphatase，ALP）（

0.380

，P=

0.003）、总胆红素（total bilirubin，TBil）（

=0.330，

=0.010）和直接胆红素（direct bilirubin，DBil）（

=0.371，

=0.004）呈正相关性，与凝血酶原活动度（prothrombin activity，PTA）（

=-0.329，

=0.036）和胆碱酯酶（cholinesterase，CHE）（

=-0.518，

0.001）呈负相关。PBC合并肝硬化患者血清GP73水平明显高于未合并患者（

=0.002）。抗线粒体抗体M2亚型抗体（anti-mitochondrial antibodies-M2，AMA-M2）、抗三联体抗体（anti-BCOADC-E2PDC-E2 OGDC-E2 antibodies，anti-3E/BPO）、抗可溶性酸性磷酸化核蛋白SPl00抗体（anti-SPl00 antibodies，anti-SP100）和抗核包膜蛋白gp210抗体（anti-gp210 antibodies，anti-gp210）阳性组与阴性组之间GP73含量差异无统计学意义。PBC患者经保肝、改善胆汁淤积等治疗后，血清AST、ALP、γ-GGT和GP73水平明显降低（

结论

GP73在PBC的诊断、疾病进程和疗效监测方面都发挥了重要作用，有望成为PBC的潜在疾病标志物。

Abstract

Objective

To investigate the role of Golgi protein 73（GP73） in the diagnosis of primary biliary cholangitis（PBC） and its association with disease progression and therapeutic efficacy monitoring.

Methods

Serum samples were collected from 70 PBC patients，36 patients with liver diseases other than autoimmune liver disease（non-AILD group），and 40 healthy controls（HC group），and ELISA was used to measure the serum level of GP73. For the inpatients with PBC，serum samples were collected before and after treatment to measure GP73.

Results

There was a significant difference in the distribution of serum GP73 concentration between the PBC group，the non-AILD group，and the HC group（

0.001），and the receiver operating characteristic（ROC） curve showed that GP73 had an area under the ROC curve of 0.839 in the diagnosis of PBC. Serum GP73 level was positively correlated with aspartate aminotransferase（AST）（

=0.337，

=0.009），alkaline phosphatase（ALP）（

=0.380，

=0.003），total bilirubin（

=0.330，

=0.010），and direct bilirubi

n（

=0.371，

=0.004），while it was negatively correlated with prothrombin activity（

=-0.329，

=0.036） and cholinesterase（

=-0.518，

0.001）. The PBC patients with liver cirrhosis had a significantly higher serum GP73 level than those without liver cirrhosis （

=0.002）. There was no significant difference in GP73 content between the patients with positive anti-mitochondrial antibodies-M2，anti-BCOADC-E2PDC-E2 OGDC-E2 antibodies，and anti-SPl00 antibodies and those with negative antibodies. The PBC patients had significant reductions in the serum levels of AST，ALP，gamma-glutamyl transpeptidase，and GP73 after liver-protecting treatment and improvement in cholestasis（

0.05）.

Conclusion

GP73 plays an important role in the diagnosis，disease progression，and efficacy monitoring of PBC and is expected to become a potential disease marker for PBC.
