Research progress on intraoperative anastomosis testing techniques to prevent anastomotic leakage after gastric cancer surgery
Anastomotic leakage is one of the most critical complications after radical gastrectomy for gastric cancer, which seriously impairs patients' rehabilitation process and can be life-threatenin. The existing studies predominantly focus on the "early detection and management" of postoperative anastomotic leakage, whereas discussions on intraoperative preventive interventions remain notably limited. During surgery, techniques such as air leak test, methylene blue leak test, indocyanine green fluorescence imaging, and gastroscopy examination can be employed to assess the structural integrity and tissue perfusion of the anastomosis in real time. It facilitates the timely repair of potential defects before fascial closure, thereby preventing the occurrence of postoperative anastomotic leakage. However, these technologies have not yet formed a unified judgment standard, and there remains a substantial risk of false negatives. Therefore, this study intends to systematically analyze and integrate the existing intraoperative anastomotic testing techniques, explore their standardized operational procedures and diagnostic thresholds to establish a more reliable intraoperative prevention system, with the ultimate goals of reducing the incidence of anastomotic leakage and improving the long-term prognosis of patients.
