Analysis of risk factors for early postoperative infection in patients with blunt splenic injury and establishment of predictive model
To analyze the risk factors for early postoperative infection in patients with blunt splenic injury, and construct a nomogram prediction model.
Methods
2
A retrospective analysis was conducted on the clinical data of 67 patients with blunt splenic injury admitted to our hospital from February 2022 to May 2025.Patients were divided into the infection group (15 cases) and the non-infection group (52 cases) according to the presence of postoperative infection.General data including age, American Association for the Surgery of Trauma (AAST) grade, injury severity score (ISS), prealbumin level at admission, activated partial thromboplastin time at admission and presence of shock, as well as perioperative indicators including surgical method, interval from admission to operation, hospital stay and postoperative bed rest time were
compared between the two groups. Univariate analysis and
Logistic
regression analysis were used to identify independent risk factors for early postoperative infection, and a nomogram prediction model was constructed accordingly.
Results
2
All 67 patients achieved successful treatment. The incidence of early postoperative infection was 22.388% (15/67), accounting for 88.235% (15/17) of all complications, among which pulmonary infection was the most common (12 cases). There were statistically significant differences in the age, mechanism of injury, ISS, AAST grade, prealbumin level at admission, activated partial thromboplastin time at admission, interval from admission to operation and hospital stay between the two groups (
P
<
0.05). Univariate
Logistic
regression analysis indicated that the age (
OR
=1.054, 95%
CI
:1.008 to 1.102,
P
=0.022), ISS (
OR
=1.171, 95%
CI
:1.049 to 1.308,
P
=0.005) and prealbumin level at admission (
OR
=0.986, 95%
CI
: 0.975 to 0.997,
P
=0.016) were risk factors for early postoperative infection. Multivariate
Logistic
regression analysis revealed that high ISS (
OR
=1.159, 95%
CI
: 1.032 to 1.303,
P
=0.013) and low prealbumin level at admission(
OR
=0.987, 95%
CI
: 0.975 to 0.999,
P
=0.036) were independent risk factors for early postoperative infection.
Conclusion
2
Infection is the predominant early complication after surgery for blunt splenic injury. High ISS and low prealbumin level at admission are independent risk factors for early postoperative infection. The nomogram model constructed based on these factors facilitates early identification of high‑risk patients and enables timely intervention.
