摘要

目的

探究腹腔镜胰腺癌手术患者术后发生静脉血栓栓塞症（VTE）的相关高危因素，构建列线图预测模型并评估其效能。

方法

选取2023年1月至2024年12月我院收治的腹腔镜胰腺癌手术后发生VTE的患者43例作为观察组，另按1∶2匹配同期未发生VTE的腹腔镜胰腺癌手术患者86例作为对照组。对2组患者的临床资料进行回顾性分析，通过多因素

Logistic

回归分析筛选危险因素，利用R软件构建列线图预测模型，结合受试者工作特征（

ROC

）曲线、校准曲线、决策曲线评估模型效能。

结果

多因素

回归分析显示，淋巴细胞计数降低（

=238.331，95%

：27.882～303.220）、组织因子升高（

=6.211，95%

：3.252～11.865）、术后第1天D-二聚体升高（

=2.744，95%

：1.506～5.000）、同种异体血管置换（

=2.622，95%

：1.113～6.175）、低分化程度（

=3.007，95%

：1.264～7.153）均为腹腔镜胰腺癌手术患者术后发生VTE的独立危险因素（

0.05）。

ROC

曲线分析显示，联合各指标预测VTE的曲线下面积（

AUC

）为0.984（95%

：0.967~1.000）；校准曲线经

拟合优度检验

²=3.543、

=0.896，提示模型预测概率与VTE发生实际概率无显著差异，拟合性良好；决策曲线分析显示，模型在风险阈值大于0.03时具有明显正向净收益，临床实用性较好。

结论

基于淋巴细胞计数、组织因子、术后第1天D-二聚体、血管重建方式、分化程度等危险因素构建的列线图模型对腹腔镜胰腺癌手术患者术后发生VTE的预测效能较高，可辅助临床识别高风险患者，为制定针对性干预措施提供依据。

Abstract

Objective

To explore the high-risk factors for postoperative venous thromboembolism （VTE） in patients undergoing laparoscopic pancreatic cancer surgery and construct a nomogram prediction model to evaluate its efficacy.

Methods

A total of 43 patients who developed VTE after laparoscopic pancreatic cancer surgery in our hospital from January 2023 to December 2024 were selected as the observation group，and 86 patients who underwent laparoscopic pancreatic cancer surgery during the same period without VTE were matched at a 1∶2 ratio as the control group.The cl

inical data of patients in the two groups were retrospectively analyzed.Independent risk factors were screened by multivariate

Logistic

regression analysis，and a nomogram prediction model was constructed by R software.The model efficacy was evaluated by

receiver operating characteristic

） curve，calibration curve，and decision curve.

Results

Multivariate

regression analysis showed that decreased lymphocyte count（

：27.882 to 303.220），elevated tissue factor （

：3.252 to 11.865），elevated D-dimer on postoperative day 1 （

：1.506 to 5.000），allogeneic vascular replacement （

=2.622，95%

：1.113 to 6.175），and poor differentiation （

=3.007，95%

：1.264 to 7.153） were the independent risk factors for postoperative VTE in patients underwent laparoscopic pancreatic cancer surgery （

0.05）.

ROC

curve analysis showed that the

area under the curve

（

AUC

） of the combined indicators for predicting postoperative VTE was 0.984 （95%

：0.967 to 1.000）.The calibration curve was tested by

Hosmer

Lemeshow

goodness-of-fit test （

²=3.543，

=0.896），indicated that there was no significant difference between the predicted probability of the model and the actual probability of VTE occurrence，with good fitting performance.Decision curve analysis showed that the model had a significant positive net benefit when the risk threshold was greater than 0.03，demonstrating favorable clinical utility.

Conclusion

The nomogram model constructed based on decreased lymphocyte count， tissue factor，D-dimer on postoperative day 1，vascular reconstruction method，and degree of differentiation demonstrates high predictive efficacy for postoperative VTE in patients undergoing laparoscopic pancreatic cancer surgery，which can assist in clinical identification of high-risk patients and provide evidence for formulating targeted intervention measures.
