Application effect of tranexamic acid combined with liposomal bupivacaine in hip arthroplasty
To investigate the effects of tranexamic acid combined with liposomal bupivacaine on analgesic effect, coagulation function, and blood loss in patients after hip arthroplasty.
Methods
2
Patients scheduled for hip arthroplasty at our hospital from October 2024 to April 2025 were randomly divided into the control group and the observation group, with 50 cases in each group. The control group received ropivacaine for incision closure, and the observation group received liposomal bupivacaine plus tranexamic acid for intra-articular instillation. The surgical indicators, analgesic effects, fibrinolytic and inflammatory indicators [D-dimer,C-reactive protein(CRP),interleukin-6(IL-6)],coagulation function [(activated partial thromboplastin time, APTT),(prothrombin time, PT),(thrombin time, TT),(fibrinogen, FIB)],and Montreal cognitive assessment (MoCA) scores were compared between two groups.
Results
2
The observation group had lower intraoperative blood loss and shorter time to first ambulation after surgery than the control group, with statistically significant differences (
P
<
0.05). The observation group had significantly lower visual analogue scale (VAS) scores within 48 hours postoperatively and significantly higher MoCA scores 24 hours after surgery compared with the control group, with statistically significant differences (
P
<
0.05). The observation group had lower levels of D-dimer,CRP,IL-6, shorter APTT,PT,and TT,and a higher level of FIB compared with the control group, with statisticall
y significant differences(
P
<
0.05).
Conclusion
2
The combination of tranexamic acid and liposomal bupivacaine in hip arthroplasty effectively reduces intraoperative blood loss, significantly improves short‑term postoperative analgesia effect, reduces fibrinolytic activity, alleviates inflammatory responses, optimizes coagulation function, and has positive effect on early postoperative cognitive recovery. It is a safe and effective perioperative management strategy.
