Prospective randomized controlled study on 3D printing-assisted pre-fenestration TEVAR for Stanford type B aortic dissection involving the left subclavian artery
To investigate the efficacy of 3D printing-assisted pre-fenestration thoracic endovascular aortic repair (TEVAR) in the treatment of Stanford type B aortic dissection (TBAD) involving the left subclavian artery.
Methods
2
A total of 190 patients with TBAD involving left subclavian artery in Suining Central Hospital from January 2020 to December 2024 were selected and divided into in situ fenestration group (95 cases) and 3D pre-fenestration group (95 cases) according to the random number table method. Patients in the in situ fenestration group were treated with in situ fenestration TEVAR, and patients in the 3D pre-fenestration group were treated with 3D printing-assisted pre-fenestration TEVAR.The operation related indexes, operation effect, occurrence of endoleak and other complications were compared between the two groups.
Results
2
The fluoroscopy time and total radiation dose area product of patients in the 3D pre-fenestration group were less
than those in the in situ fenestration group (
P
<
0.05). There were no significant differences in operation time, length of anchor area, successful release of aortic stent and success rate of left subclavian artery reconstruction, success rate of operation, occurrence of endoleak, rate of secondary operation, and incidence of other complications between the two groups (
P
>
0.05).
Conclusion
2
Both 3D printing-assisted pre-fenestration and in-situ fenestration TEVAR treatments for TBAD involving the left subclavian artery demonstrate good surgical outcomes, and the 3D printing-assisted pre-fenestration technique can reduce intraoperative fluoroscopy time and radiation dose.
