Effect of preoperative nutritional status on postoperative pulmonary infection in elderly patients undergoing hip arthroplasty with invasive mechanical ventilation
To clarify the impact of preoperative nutritional status on postoperative pulmonary infection in elderly patients undergoing hip arthroplasty with invasive mechanical ventilation.
Methods
2
A total of 249 elderly patients who received hip arthroplasty under general anesthesia with invasive mechanical ventilation at Suqian Hospital of Jiangsu Province People's Hospital were enrolled in this study. According to the mini nutritional assessment short form (MNA-SF) scores evaluated 1 day before surgery, patients with scores ranging from 12 to 14 points were assigned to the normal group (
n
=158),and those with scores below 12 points were assigned to the low-score group (
n
=91).The levels of hemoglobin (Hb),albumin (ALB), prealbumin (PA), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), white blood cell count (WBC), C-reactive protein (CRP), and neutrophil percentage (NP) before and after surgery were compared between the two groups. The two groups were compared in terms of the occurrence of postoperative pulmonary infection and related symptoms.
Results
2
The levels of Hb,ALB,and PA 1 day before surgery and 1 day after surgery in the low-score group were significantly lower than those in the normal group (
P
0.05);the levels of TNF-α, IL-6, WBC, CRP, and NP 1 day after surgery in the low-score group were significantly higher than those in the normal group (
P
0.05). The proportions of patients with body temperature38 ℃, purulent airway secretions, abnormal WBC, pulmonary moist rales or consolidation signs, and postoperative pulmonary infection after surgery in the low-score group were significa
ntly higher than those in the normal group(
P
0.05).
Conclusion
2
Elderly patients with low preoperative nutritional scores have poor resistance, and are prone to inflammation and infection after undergoing hip arthroplasty with invasive mechanical ventilation, thus increasing the risk of pulmonary infection.
