Comparison on the efficacy of ultrasound-guided radiofrequency endovenous obliteration and endovenous microwave ablation in the treatment of lower extremity varicose veins
To compare the efficacy of ultrasound-guided radiofrequency endovenous obliteration (RFO) and endovenous microwave ablation (EMWA) in the treatment of lower extremity varicose veins.
Methods
2
A retrospective selection of 120 patients with lower extremity varicose veins admitted to our hospital was conducted, and they were divided into the RFO group and the EMWA group according to different surgical methods, with 60 cases in each group. Patients in the RFO group received ultrasound-guided RFO, while patients in the EMWA group received ultrasound-guided EMWA. The efficacy, perioperative indicators, venous clinical severity score (VCSS), chronic venous insufficiency questionnaire (CIVIQ), heart rate (HR), percutaneous arterial oxygen saturation (SpO
2
), mean arterial pressure (MAP), matrix metallopeptidase-13 (MMP-13), tumor necrosis factor-alpha (TNF-α), and the incidence of complications were compared between the two groups.
Results
2
There was no statistically significant difference in the total effective rate between the two groups of patients (
P
>
0.05). Patients in the EMWA group had lower intraoperative blood loss (
P
<
0.05), shorter hospital stay (
P
<
0.05), and longer operation time and postoperative ambulation time compared to the RFO group (
P
<
0.05). There was no statistically significant difference in VCSS or CIVIQ scores before surgery and 6 months after surgery between the two groups of patients (
P
>
0.05). There was no statistically significant difference in HR, SpO
2
, or MAP 1
2 hours after surgery between the two groups of patients (
P
>
0.05). There was no statistically significant difference in MMP-13 level or TNF-α level 7 days after surgery between the two groups of patients (
P
>
0.05). There was no statistically significant difference in the total incidence of complications between the two groups of patients (
P
>
0.05).
Conclusion
2
The efficacy of ultrasound-guided RFO and EMWA is comparable in the treatment of lower extremity varicose veins. Both can effectively reduce disease severity, inhibit systemic inflammatory response, improve patients' quality of life, and demonstrate high safety. EMWA is superior in reducing intraoperative blood loss and shortening hospital stay, while RFO is superior in shortening operative time and accelerating early postoperative activity. Clinically, the surgical procedure can be reasonably selected based on the patient's individual disease status, physical condition, and treatment needs.
