Effects of different CO₂ pneumoperitoneum pressures on clinical outcomes of laparoscopic bariatric surgery in obese patients
To evaluate the effects of different CO₂ pneumoperitoneum pressures on acute lung injury, recovery quality, and pain in obese patients undergoing laparoscopic bariatric surgery.
Methods
2
A total of 70 obese patients who underwent laparoscopic sleeve gastrectomy bariatric surgery at our hospital were selected as the study subjects and randomly divided into the high-pressure group (
n
=35) and the low-pressure group (
n
=35), with intraoperative CO₂ pneumoperitoneum pressure at 16 mmHg and 12 mmHg, respectively. The surgical conditions, respiratory mechanics indicators, inflammatory factor levels, general anesthesia recovery status, and pain status were compared between the two groups.
Results
2
There were no statistically significant differences between the two groups in terms of operation time, intraoperative blood loss, intraoperative dosage of propofol, intraoperative dosage of remifentanil, and intraoperative dosage of rocuronium bromide (
P
0.05).At 60 minutes of operation, the peak airway pressure (Ppeak) and plateau airway pressure (Pplat) of patients in the low-pressure group were significantly lower than those in the high-pressure group (
P
0.05). Immediately after mechanical ventilation, the levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) of patients in the low-pressure group were significantly lower than those in the high-pressure group (
P
0.05).The time to spontaneous breathing recovery, awakening time, tracheal extubation time, and length of stay in the post-anesthesia care unit of patients in the low-pressure group were significantly shorter than those in the high-pressure group (
P
0.05).At 12 and 24 hours after surgery, pain scores of patients in the low-pressure group were significantly lower than those in the high-pressure group (
P
0.05).The dosage of sufentanil for rescue analgesia within 48 hours after surgery of patients in the low-pressure group was significantly less than that in the high-pressure gro
up (
P
0.05).
Conclusion
2
For obese patients undergoing laparoscopic bariatric surgery, compared with a CO₂ pneumoperitoneum pressure of 16 mmHg, 12 mmHg CO₂ pneumoperitoneum pressure does not increase the difficulty of surgery or the dosage of anesthetic drugs, resulting in better recovery quality, milder acute lung injury, and less postoperative pain.
