Clinical value of narrow band imaging assisted thulium fiber laser en-bloc resection for non-muscle-invasive bladder cancer
To explore the safety and clinical efficacy of transurethral narrow band imaging (NBI) assisted thulium fiber laser (TFL) en-bloc resection of bladder tumor (ERBT) in the treatment of non-muscle-invasive bladder cancer.
Methods
2
A retrospective analysis was performed on 260 patients with non-muscle-invasive bladder cancer admitted to People's Hospital of Inner Mongolia Autonomous Region from January 2021 to December 2023. Seventy-six patients treated with transurethral narrow band imaging assisted thulium fiber laser en-bloc resection of bladder tumor were enrolled in the NBI-TFL-ERBT group, and 184 patients treated with conventional transurethral resection of bladder tumor (TURBT) under white light imaging (WLI) were enrolled in the WLI-TURBT group. The operation time, postoperative catheterization time, postoperative hospital stay, incidence of surgical complications, detection rate of muscular layer in pathological specimens, recurrence rate and disease-free survival rate were statistically compared between the two groups.
Results
2
There were no significant differences between the two groups in terms of operative time, postoperative hospital stay, and the incidence of bladder hemorrhage (
P
>
0.05).Compared with the WLI-TURBT group, patients of the NBI-TFL-ERBT group had shorter postoperative cathe
terization time (
P
<
0.05), lower incidences of obturator nerve reflex, bladder perforation and overall complications (
P
<
0.05), and higher detection rate of muscular layer in pathological specimens (
P
<
0.05).Follow-up results showed that compared with the WLI-TURBT group, patients of the NBI-TFL-ERBT group had lower tumor recurrence rates at 6 months, 1 year, 2 years after surgery and at the end of follow-up (
P
<
0.05), with a higher disease-free survival rate (
P
<
0.05).
Conclusion
2
Transurethral narrow band imaging assisted thulium fiber laser en-bloc resection of bladder tumor is safe and feasible in the treatment of non-muscle-invasive bladder cancer. Compared with the conventional transurethral resection of bladder tumor, this technique can significantly reduce the risk of complications, promote postoperative recovery and improve survival benefits of patients.
