摘要

目的

探究比伐芦定在急性冠状动脉综合征患者行冠脉旁路移植术（CABG）中的应用效果。

方法

选取我院于2023年5月至2024年5月180例因急性冠状动脉综合征行CABG的患者，随机分为对照组（采用阿司匹林和普通肝素治疗）和观察组（采用阿司匹林联合比伐芦定治疗），每组90例。比较2组患者心肌灌注指标、心功能指标、血流恢复情况、内皮功能指数、炎症因子及术后不良反应发生率。

结果

观察组患者冠状动脉心肌梗死溶栓治疗（TIMI）血流分级2级占比明显低于对照组（

0.05），3级占比显著高于对照组（

0.05）。治疗后2组患者左心室舒张末期内径（LVEDD）、左心室舒张末期容积（LVEDV）及内皮素（ET-1）、超敏C反应蛋白（hs-CRP)、肿瘤坏死因子α（TNF-α）水平均显著小/低于治疗前和对照组（

0.05）。治疗后2组患者左心室射血分数（LVEF）、心脏指数（CI）及一氧化氮（NO）、一氧化氮合酶（NOS）水平大/高于对照组和治疗前（

0.05）。观察组患者慢复流或无复流占比及治疗期间药物不良反应发生率显著低于对照组（

0.05）。

结论

对于行CABG的急性冠状动脉综合征患者，阿司匹林联合比伐芦定在促进心脏灌注、改善心功能指标和减少血管内皮功能损伤等方面效果显著，且安全性高。

Abstract

Objective

To explore the application effect of bivalirudin in patients with acute coronary syndrome undergoing coronary artery bypass grafting （CABG）.

Methods

A total of 180 patients who underwent CABG for acute coronary syndrome at our hospital from May 2023 to May 2024 were enrolled and randomly assigned to the control group （received aspirin and unfractionated heparin） and the observation group （received aspirin combined with bivalirudin）， with 90 cases in each group. A comparison was conducted between the two groups regarding myocardial perfusion parameters， cardiac function parameters， blood flow recovery status， endothelial function index， levels of inflammatory factors， and incidence of postoperative adverse reactions.

Results

The observation group had a significantly lower proportion of TIMI flow grade 2 and a higher proportion of grade 3 compared to the control group （

The left ventricular end diastolic diameter （LVEDD），left ventricular end diastolic volume （LVEDV），and levels of endothelin-1 （ET-1），hypersensitive C-reactive protein （hs-CRP），and tumor necrosis factor-α（TNF-α）after treatment in the observation group were smaller/lower than those before treatment and in the control group （

0.05）. The left ventricular ejection fraction （LVEF），cardiac index （CI），and levels of nitric oxide （NO），and nitric oxide synthase （NOS）after treatment in the observation group were greater/higher than those in the control group and before treatment （

0.05）. The proportion of patients with slow or no reperfusion and the incidence of adverse drug reactions during treatment in the observation group were significantly lower than those in the control group （

0.05）.

Conclusion

For patients undergoing CABG for acute coronary syndrome，the combination of aspirin and bivalirudin significantly promotes cardiac perfusion，improves cardiac function indicators， reduces vascular endothelial injury，and demonstrates a high safety.
