摘要

目的

探究股骨远端90°锁定钢板治疗儿童股骨髁上骨折的临床效果。

方法

选取2022年1月至2023年10月内江市中医医院收治的150例股骨髁上骨折患儿，采用随机数字表法分为对照组（

=75）和观察组（

=75）。对照组患儿采用闭合复位克氏针固定，观察组患儿采用股骨远端90°锁定钢板固定。比较2组患儿围术期情况、术后恢复情况、手术前后疼痛视觉模拟量表（VAS）评分、影像学指标、术后膝关节功能[美国特种外科医院（HSS）评分

]

、膝屈-伸活动度（ROM）、术后并发症、膝关节恢复优良率。

结果

观察组患儿术中透视次数少于对照组，手术时间、首次下床行走时间、骨折愈合时间短于对照组（

0.05）。观察组患儿术后2周、1个月VAS评分低于对照组（

0.05）。观察组患儿术后3个月、6个月股骨长度差小于对照组（

0.05）；观察组患儿术后3个月、6个月HSS评分、膝屈-伸ROM高/大于对照组（

0.05）。观察组患儿并发症发生率低于对照组（

0.05）。术后1年，观察组患儿膝关节恢复优良率高于对照组（

0.05）。

结论

股骨远端90°锁定钢板治疗儿童股骨髁上骨折能优化手术流程，减轻疼痛，促进骨折愈合，恢复股骨髁上骨形态，减少并发症发生，临床效果较好。

Abstract

Objective

To investigate the clinical efficacy of the distal femur 90° locking plate in the treatment of pediatric supracondylar femoral fractures.

Methods

A total of 150 children with supracondylar femoral fractures in Neijiang Hospital of Traditional Chinese Medicine from January 2022 to October 2023 were enrolled，and assigned into the control group （

=75） and the observation group （

=75） by random number table method. Patients in the control group were treated with closed reduction and Kirschner wire fixation，while patients in the observation group were treated with the distal femur 90° locking plate fixation. The perioperative conditions，postoperative recovery，visual analogue scale （VAS） scores for pain before

and after surgery，imaging indicators，postoperative knee joint function ［hospital for special surgery （HSS） score］，knee extension-flexion range of motion （ROM），postoperative complications，and the excellent and good rate of knee joint recovery of patients between the two groups were compared.

Results

The number of intraoperative fluoroscopy of patients in the observation group was less than that in the control group，and the operation time，time to ambulation and fracture healing time were shorter than those in the control group （

0.05）. The VAS scores of patients in the observation group 2 weeks and 1 month after surgery were lower than those in the control group （

0.05）. The femoral length discrepancies of patients in the observation group 3 months and 6 months after surgery were smaller than those in the control group （

0.05）. The HSS score and knee extension-flexion ROM of patients in the observation group 3 months and 6 months after surgery were higher/greater than those in the control group（

0.05）. The incidence of complications in the observation group was lower than that in the control group（

0.05）. One year after surgery，the excellent and good rate of knee joint recovery in the observation group was higher than that in the control group（

0.05）.

Conclusion

The distal femur 90° locking plate in the treatment of supracondylar femoral fractures in children can optimize the surgical process，relieve pain，promote fracture healing，restore the morphology of the supracondylar bone of the femur，and reduce the occurrence of complications，with good clinical effects.
