Comparison of diagnostic value of DWI-MRI and PET in lymph node metastasis of esophageal cancer
To compare the diagnostic value of diffusion-weighted magnetic resonance imaging (DWI-MRI) and positron emission tomography (PET) in the lymph node metastasis in esophageal cancer.
Methods
2
A retrospective analysis was performed on the clinical data of 100 patients with esophageal squamous cell carcinoma admitted to our hospital from April 2019 to April 2023.All patients underwent DWI-MRI and PET examinations. The differences in the diagnostic accuracy for clinical staging, the accuracy for evaluating lymph node metastasis (including sensitivity, specificity, positive predictive value, negative predictive value and overall accuracy), and the accuracy for assessing the relevant characteristics of metastatic lymph nodes (including nodal location, nodal size, intranodal cancer nest size and the area occupied by intranodal cancer nests) between the two examination methods were compared.
Results
2
There was no statistically significant difference in the diagnostic accuracy of clinical staging between the two examination methods (
P
>
0.05). However, the overestimation rate of DWI-MRI was higher than that of PET (
P
<
0.05), and its underestimation rate was lower than that of PET (
P
<
0.05).A total of 1617 lymph nodes were collected from the 100 patients, among which 124 were metastatic. Perigastric abdominal and costal lymph node metastasis was the most common type (57 nodes, 45.97%).There was no statistically significant difference in the accuracy of evaluating lymph node metastasis between the two examination methods (
P
>
0.05).The sensitivity and negative predictive value of DWI-MRI were higher than those of PET (
P
<
0.05),while its specificity and positive predictive value were lower than those of PET (
P
<
0.05).There was no statistically significant difference in the diagnostic accuracy between the two examination methods in evaluating the location of metastatic lymph nodes (middle and lower thoracic region, abdominal and costal regions), lymph node size
<
5 mm, and the area occupied by intranodal cancer nests
<
1/3 (
P
>
0.05). However, DWI-MRI showed a significantly hi
gher diagnostic accuracy than PET in assessing the location of metastatic lymph nodes (cervical and upper thoracic regions), lymph node size≥5 mm, intranodal cancer nest size (
<
5 mm, 5~10 mm,
>
10 mm), and the area occupied by intranodal cancer nests≥1/3 (
P
<
0.05).
Conclusion
2
Both DWI-MRI and PET have certain diagnostic value in the evaluation of lymph node metastasis in esophageal cancer, while DWI-MRI demonstrates higher accuracy in assessing the relevant characteristics of lymph node metastasis in the cervical and upper thoracic regions.
