Application of transvaginal natural orifice transluminal endoscopic surgery in pelvic organ prolapse
To investigate the application effect of transvaginal natural orifice transluminal endoscopic surgery (V-NOTES) in patients with pelvic organ prolapse.
Methods
2
A total of 120 patients with pelvic organ prolapse who were hospitalized in the Gynecology Department of Urumqi Youai Hospital from January 2021 to April 2024 were selected and randomly divided into the observation group and the control group, with 60 cases in each group. Patients in the observation group received V-NOTES, while those in the control group underwent multi-port laparoscopic surgery. The time of access establishment, operation time, intraoperative blood loss, pelvic adhesion status, blood transfusion requirement, anal exhaust status 48 hours after surgery, the degree of wound pain 1 day and 2 days after surgery, postoperative analgesic use, postoperative hospital stay, and fever occurrence of patients between the two groups were compared. At 1 month, 3 months, 6 months, and 1 year after surgery, the pelvic organ prolapse quantification (POP-Q), patient global impression of improvement (PGI-I), pelvic floor distress inventory-short form 20 (PFDI-20), and pelvic floor impact questionnaire-short form 7 (PFIQ-7) were used to evaluate objective efficacy, subjective efficacy, and the impact of pelvic floor symptoms on patients, respectively.
Results
2
There was no statistically significant difference in time of access establishment, operation time, intraoperative blood loss, or pelvic adhesion status between the two groups (
P
>
0.05). The proportion of patients with anal exhaust within 48 hours after surgery in the observation group was significantly higher than that in the control group (
P
<
0.05), and the proportion of patients using postoperative analgesics was signifi
cantly lower than that in the control group (
P
<
0.05). No statistically significant differences were found in the objective effective rates between the two groups at 1 month, 3 months, 6 months, or 1 year after surgery (
P
>
0.05). At 6 months and 1 year after surgery, the PGI-I score, PFDI-20 score, and PFIQ-7 score in the observation group were significantly lower than those in the control group (
P
<
0.05).
Conclusion
2
Both V-NOTES and multi-port laparoscopic surgery have good effects in the treatment of female pelvic organ prolapse. V-NOTES is associated with earlier postoperative anal exhaust, milder pain, better subjective feelings, and superior long-term pelvic floor function recovery in patients.
