Pathogenic bacteria features and preoperative nutrition‑related risk factors for pulmonary infection after emergency endovascular thrombectomy in stroke patients
To investigate the incidence, distribution of pathogenic bacteria and preoperative nutrition‑related influencing factors of pulmonary infection after emergency endovascular thrombectomy in stroke patients.
Methods
2
A total of 196 elderly patients who underwent emergency endovascular thrombectomy for acute ischemic stroke at Suqian Hospital of Jiangsu Province People's Hospital from July 2022 to June 2025 were enrolled. They were divided into the infection group (
n
=47) and the non-infection group (
n
=149) according to the occurrence of postoperative pulmonary infection. The distribution of pathogenic bacteria after surgery was analyzed. The preoperative nutritional indicators were compared between the two groups, and risk factors for pulmonary infection were analyzed.
Results
2
Among the 196 patients, 47 cases (23.98%) developed pulmonary infection. A total of 67 strains of pathogenic bacteria were isolated, of which 43 strains (64.18%) were Gram‑negative bacteria, representing the highest proportion. Statistically significant differences were found between the two groups in nutritional risk screening 2002 (NRS-2002) score, nutrition risk in the critically ill (NUTRIC) score, total lymphocyte count and levels of hemoglobin, total protein, and albumin (
P
<
0.05), while no statistically significant differences were observed in the remaining baseline variables (
P
>
0.05). Multivariate
Logistic
regression analysis indic
ated that NUTRIC score≥5 points, total protein
<
60 g/L and albumin
<
35 g/L were independent risk factors for pulmonary infection after emergency endovascular thrombectomy in stroke patients(
OR
>
1,
P
<
0.05).
Conclusion
2
The incidence of pulmonary infection after emergency endovascular thrombectomy in stroke patients is relatively high, and Gram-negative bacteria are the predominant pathogenic bacteria. Patients with higher preoperative NUTRIC score, and lower levels of total protein and albumin are more susceptible to postoperative pulmonary infection.
