Effects of a transitional care program for post-stroke patients on independent living ability and satisfaction with discharge care
Objective To evaluate the effects of a transitional care program for post-stroke patients on independent living ability and satisfaction with discharge care. Methods Forty-two ischemic stroke survivors
selected in Southwest Hospital from May to December 2010
were randomized to a control group (
n
=19) and a study group (
n
=23). The control group received routine discharge care
and the study group received routine discharge care plus
a transitional care program. The transitional care program included stroke health courses 1 week before discharge
comprehensive discharge assessment 3 d before discharge
telephone follow-up 1 week after discharge
and family visit 4 weeks after discharge. Outcome measures including Barthel Index
subjective life quality
and satisfaction with discharge care were used. The data were collected 3 d before discharge and 12 weeks after discharge using a self-designed structured questionnaire. Results At the end of the 12th week after discharge
the increase of Barthel Index in the study group was higher than that in the control group (18.70±9.32
vs
13.16±5.33
P
<
0.05). The improvement of subjective life quality had no significant difference between the study group and the control group (1.52±0.85
vs
0.89±1.15
P
>0.05). The higher score of satisfaction with discharge care was founded in the study group than in the control group (86.96±10.20
vs
53.16±14.55
P
<
0.01). Conclusion The transitional care program for post-stroke patients is effective in improving basic independent living ability and patient satisfaction with discharge care
and ineffective in improving subjective perception of life quality.
