摘要

对４４例原发于皮肤粘膜的恶性黑色累瘤（恶黑）进行光镜观察及ＨＭＢ４５，Ｓ—１００蛋白，Ｖｉｍｅｎｔｉｍ免疫组化研究。光镜见恶黑瘤细胞形态及细胞排列方式多种多样，易与各种低分化癌、肉瘤、淋巴瘤相混淆。ＨＭＢ４５、Ｓ—１００蛋白、Ｖｉｍｅｎｔｉｎ免疫组化检测，其阳性率分别为８８．６％，９５．５％。幻。７％。结果提示，ＨＭＢ４５对恶黑具有较高的特异性和敏感性，对恶黑的诊断有重要意义。Ｓ—１００蛋白，Ｖｉｍｅｎｔｉｎ对恶黑有较高的敏感性，两者配合使用，对恶黑的诊断有辅助作用。

Abstract

Fourty-four cases of prunary malignant melanoma(MM)were studi ed

Histologic examina-tion revealed that MM have different cellular types and arrangement of cells. This makes itdifficult to distingul sh them from various porrly differentiated carcinomas

sarcomas andlymphomas. On immunohistochemical study

the rate of positivity with HMB45

S-100 protein andvimentin was 88. 6%

95.5%and 97.7%respectively.Results indicate that HMB45 has not onlyhigher specificity

but also sensitivity for MM

which are useful in the diagnosis of MM. S-100protein and vimentin has greater sensitivity only. To use them together will give a supplementaryrole for diagnosis of MM.
