摘要

目的:总结心内直视术后多器官功能不全综合征（Multiple organ dysfunction syndrome

MODS）病人的防治体会。方法:本研究对近10年来心内直视术后未预防组和预防组发生MODS患者的临床资料根据病情轻、中、重进行分层分析

并采取Mantel-haenszel检验

对MODS的防治进行探讨。结果:未采取防治措施组与采取防治措施组

在轻度患者中无显著差异（Fisher’s确切概率法

P=0.2132

P>0.05）;在中、重度患者中

采用卡方检验（χ2=13.5641

P=0.0002和χ2=33.5149

P<0.001）均有显著性差异（χ2=49.2371

P<0.05）;合并3层做总的Mantel-haenszel检验表明（Cochran-Mantel-Haenszel卡方χ2=49.2371

P<0.001）

认为未预防组和预防组结果有差异显著。结论:术中良好的心肌保护、提高手术技巧、缩短主动脉阻断时间、加强呼吸循环及胃肠道管理、预防感染是防治MODS的关键因素。

Abstract

Objective:To Summarise the experience of treatment and prevention for multiple organ dysfunction syndrome(MODS)following cardiac operation.Methods:Patients’datea including treatment group and control group were collected and alalysed who underwent cardiac Surgery in the last ten years.Results:There is no difference between treatment group and control group in mild patients(P>0.05);There are significant differences between treatment group and control group in moderate and severe patients(P<0.005);There are significant differences between treatment group and control group in all patients(P<0.001).Conclusion:The Key factors to treat and prevent MODS are effective myocardial protection

improved operative technique

reduced aortic cross clamp time

intensive management of respiratory、Circulations and gastrointestin

prevention of infection.
