The value of contrast-enhanced ultrasonography in evaluating microcirculation of extensor pollicis brevis in the dorsum of foot in diabetic patients
Objective To evaluate the value of contrast-enhanced ultrasound (CEUS) in evaluating skeletal muscle microcirculation in Diabetic Foot Dorsum. Method 25 cases of the normal control group
14 cases of type 2 Diabetes Mellitus (DM) group
and 16 cases of type 2 diabetes mellitus with microangiopathy (DM+MC) group were collected. The blood perfusion of the extensor pollicis brevis was measured by contrast-enhanced ultrasound (CEUS)
and the perfusion parameters such as the time of arrival of contrast medium (AT)
the time from the lateral tarsal artery to the extensor pollicis Brevis (ΔAT)
the peak intensity (PI) and the actual enhancement intensity (ΔPI) were obtained
the patients with different angiographic parameters were compared with each other. Using DM+MC as the criterion
the diagnostic efficacy of angiography parameters in early microcirculation disorders was investigated. Results The difference between PI and ΔPI of CEUS in each group was statistically significant (P<0.05). There was no significant difference in AT and ΔAT between normal group and DM group (P>0.05)
but there was significant difference in AT and ΔAT between DM group and DM+MC group
normal group and DM+MC group (P<0.05). The clinical diagnosis of DM+MC showed that ΔAT (AUC 0.989) and ΔPI (AUC 0.954) of CEUS were the best. Conclusion CEUS can evaluate the microcirculation of the skeletal muscle of the dorsum of the diabetic foot. The parameters ΔAT and ΔPI can provide reliable diagnostic information for the early detection of the microcirculation disturbance of the skeletal muscle of Type 2 diabetic patients.
