Objective To investigate the anatomical features of threedimensional imaging of hilar cholangiocarcinoma
and to provide a reference for the preoperative evaluation and the formulation of surgical program for patients. Methods The image data of 103 patients with hilar cholangiocarcinoma who were admitted to the first affiliated hospital of China medical university from July 2014 to March 2018 were retrospectively analyzed. The multislice spiral CT images were converted to threedimensional images by Myrian system so as to study the anatomical characteristics. Results Among the 103 patients
the variation rate of portal vein in hilar region was 14.6%
the occurence rate of typical branch form
trifurcation type
Ishaped type was 85.4%
2.9%
11.7%
respectively. The variation rate of hilar bile ducts was 10.7%
including RPSD converging into LHD type
RPSD converging into CHD type and trifurcation type
with the incidence rate of 3.9%
3.9%
2.9%
respectively. The variation rate of hepatic artery was 21.4%
and the invasion rate of hepatic artery and portal vein were 40.0% and 13.6% respectively. Conclusion The variation of hepatic artery
portal vein and bile duct are common in hilar region.Threedimensional reconstruction can clearly show the stereoscopic anatomy of the blood vessels and dilated bile ducts in the liver and truly reflect the adjacent relationship and infiltration degree of the tumor
