Objective To investigate the effect of the online co-management care model in elderly patients with type 2 diabetes combined with coronary heart disease. Methods Seventy elderly patients with type 2 diabetes complicated with coronary heart disease who visited the Ninth People’s Hospital of Zhengzhou from February 2020 to November 2021 were randomly assigned to a control group and an experimental group

with 35 cases in each group. Patients in the control group received routine continuous care

and patients in the experimental group received the online co-management care model on top of the care in the control group.Patients in both groups were intervened for 6 months.Blood glucose level

cardiac function

quality of life

and care quality score were compared between the two groups at discharge and 6 months after discharge.Results The fasting blood glucose

glycosylated hemoglobin

and 2 h postprandial blood glucose levels at 6 months after discharge in the two groups were lower than those at discharge

and the experimental group was lower than the control group

with statistically significant differences（P<0.05）.Stroke volume

left ventricular ejection fraction and cardiac output at 6 months after discharge were higher in the 2 groups than those at discharge

with the experimental group higher than in the control group

and the differences were statistically significant（P<0.05）.The SF-36 score at 6 months after discharge was higher in the 2 groups than that at discharge

with the experimental group higher than in the control group

and the differences were statistically significant（P<0.05）. The scores of care quality such as service attitude

nursing record

health education

and basic care in the experimental group were higher than those in the control group

and the differences were statistically significant（P<0.05）. Conclusion The application of the online co-management care model in elderly patients with type 2 diabetes combined with coronary heart disease is helpful to improve the blood glucose level and cardiac function

raise the quality of life of patients and boost the quality of care.
