Effect of We Chat-based continuing care on the self-management ability of diabetic patients
Objective To explore the effect of continuing care based on the We Chat platform in the out-of-hospital care of diabetic patients. Methods Eighty-four cases of diabetic patients admitted to a hospital in Zhengzhou from January to June 2021 were selected as the study subjects and divided into an observation group and a control group using the random number table method
with 42 cases each.Patients in the control group received conventional continuing care
and patients in the observation group were given continuing care based on the We Chat platform. The scores of the Diabetes Adult Knowledge Scale and the The Summary of Diabetes Self-Care Activities Disorders(SDSCA) at the time of discharge and at 1 month of discharge were compared between the 2 groups of patients. The occurrence of complications
including hypoglycemia
diabetic ketoacidosis(DKA)
and hyperglycemic hyperosmolar state(HHS) in the 2 groups was also compared. Results At the time of discharge
there was no statistically significant difference between the scores of the Diabetes Adult Knowledge Scale in the 2 groups(P>0.05).At 1 month of discharge
the scores of the Diabetes Adult Knowledge Scale were higher than those of the patients in the 2 groups at discharge and higher in the observation group than in the control group
with statistically significant differences(P<0.05).At the time of discharge
there was no statistically significant difference in the SDSCA scores for diet control
medication compliance
foot care
regular exercise
hypoglycemia prevention and management
and blood glucose monitoring between the 2 groups(P>0.05).At 1 month of discharge
the scores for diet control
medication compliance
foot care
regular exercise
hypoglycemia prevention and management
and blood glucose monitoring in SDSCA were higher in both groups than at discharge and higher in the observation group than in the control group
with statistically significant differences(P<0.05).The complication rate of patients in the observation group was 4.76%
lower than that of 19.04% in the control group
and the difference was statistically significant(P<0.05).Conclusion The effect of continuing care based on the We Chat platform in the out-of-hospital care of diabetic patients is significant. It effectively improves the level of disease-related knowledge and self-management ability of patients and reduces the incidence of complications
and is worthy of clinical promotion and application.
