Comparison of analgesic effect and hemodynamic effect of compound lidocaine cream and lidocaine on patients with arterial puncture
Objective To compare and analyze the effects of compound lidocaine cream and lidocaine on analgesia and hemodynamics on patients with arterial puncture. Methods Eighty-nine elderly patients who underwent invasive arterial blood pressure monitoring in a hospital in Liaoning Province from January 2020 to January 2021 were selected for the study
and the patients were divided into a control group(n=44) and an observation group(n=45) using the random number table method. Before performing arterial puncture
the anesthesiologist gave local infiltration anesthesia with lidocaine to the control group patients. Patients in the observation group were given local anesthesia with compound lidocaine cream for cortical layer. The visual analog scale(VAS) scores of the 2 groups of patients at several time points after puncture were compared. The hemodynamic profiles
including postextrasystolic potentiation(PEP)
cardiac output(CO)
heart rate(HR)
cardiac index(CI)
mean arterial pressure(MAP)
and stroke volume(SV) of patients in the 2 groups at several time points after receiving different protocols of anesthesia were compared. Results There was no statistically significant difference in VAS scores at 4 h
6 h
and 12 h after puncture between the 2 groups(P>0.05). At the time of puncture
there was no statistically significant difference between the levels of PEP
CO
HR
CI
MAP
and SV in the 2 groups(P>0.05). At 4 h after puncture
the levels of HR
CI
CO
and MAP in the 2 groups were lower than those at the time of puncture
while those in the observation group were higher than those in the control group
and the differences were all statistically significant(P<0.05). At 4 h after puncture
PEP was shorter than at the time of puncture in the 2 groups
while it was longer in the observation group than in the control group
and the difference was statistically significant(P<0.05). At 4 h after puncture
SV was smaller in the 2 groups than at the time of puncture
while it was larger in the observation group than in the control group
and all differences were statistically significant(P<0.05). Conclusion Both compound lidocaine cream and lidocaine local infiltration anesthesia can be used for patients with invasive arterial blood pressure monitoring
but compared with lidocaine local infiltration anesthesia
compound lidocaine cream is more effective and safer for maintaining hemodynamic stability of patients and has higher value for clinical promotion.
