Relationship between EEG network changes and executive function in patients with unilateral temporal lobe epilepsy
Objective To investigate the relationship between the changes of EEG network and executive function in patients with unilateral temporal lobe epilepsy(TLE). Methods A total of 87 patients with unilateral TLE treated in the epilepsy clinic of a hospital in Liaoning Province from March 2017 to March 2020 were divided into left L-TLE group(n=43) and R-TLE group(n=44) according to the location of the lesions. Healthy volunteers who went to the hospital for physical examination in the same period were selected as control group(n=28). The subjects were examined by functional magnetic resonance imaging(fMRI) and executive function test. The general data
the resting state functional connection differences of the default mode network(DMN) and the results of executive function test of the three groups were compared. The correlation between the functional connection value(z FC) of differential brain regions converted by Fisher-z and the efficiency of executive network in patients with TLE was analyzed. Results In L-TLE group
the descending brain regions of DMN resting functional connections were right superior frontal gyrus
bilateral middle frontal gyrus
left middle occipital gyrus and left talar cleft cortex. The enhanced brain regions were left middle temporal gyrus and left superior frontal gyrus. In R-TLE group
the descending brain regions of DMN resting functional connections were right anterior cingulate gyrus
left middle cingulate gyrus and right precuneus. The enhanced brain regions were left precuneus
right inferior parietal lobule and left inferior parietal lobule. The response time towards the same orientation
the reaction time to double orientation and the efficiency of execution network in L-TLE group and R-TLE group were significantly higher than those in control group(P<0.05). The network efficiency in R-TLE group was significantly lower than that in L-TLE group(P<0.05). In L-TLE group
the z FC value of left superior frontal gyrus was positively correlated with the efficiency of executive network(r=0.624
P=0.023). Conclusion Unilateral TLE patients have abnormal DMN resting state functional connections and abnormal executive dysfunction. The distribution of abnormal DMN resting state functional connections in L-TLE patients is wider than that in R-TLE patients
the executive dysfunction is more serious
and the changes of EEG network are related to executive dysfunction.
