Analysis of pathogenic characteristics with sepsis caused by intra-abdominal infection in children
Objective To investigate the pathogenic bacteria composition and drug resistance in sepsis caused by intra-abdominal infection in children in Jinan
Shandong province
China
and to provide a reference for the clinical treatment. Methods A retrospective analysis was performed on the case data of pediatric patients with sepsis caused by intra-abdominal infection who were hospitalized in the Pediatric Intensive Care Unit in Jinan People’s Hospital Affiliated to Shandong First Medical University from January 2018 to December 2019. Results A total of 107 strains of pathogenic bacteria were isolated from 70 pediatric patients
which were mainly Gram-negative bacteria(77.57%
83 strains)
and the other 24 strains(22.43%)were Gram-positive bacteria. The top three pathogens were Escherichia coli(56.07%)
Pseudomonas aeruginosa(12.15%)
and Streptococcus constellatus(5.61%).Among the Escherichia coli strains
22(36.67%)were extended-spectrum β-lactamases-producing strains. Over75% of the strains were not sensitive to cefazolin
ampicillin
piperacillin
and other β-lactam antibacterial drugs.Over 30% of the strains were resistant to cefuroxime
ceftriaxone
cefotaxime
cefepime
and other cephalosporins. The strains had a low resistance rate to compound formulations of β-lactam antibiotics
e.g.
piperacillin-tazobactam
and were sensitive to carbapenem antibacterial drugs. The Pseudomonas aeruginosa strains had a resistance rate of15.38% to aztreonam
and were sensitive to other commonly used drugs. Conclusion Gram-negative bacteria such as Escherichia coli and Pseudomonas aeruginosa are the main pathogens in sepsis caused by intraabdominal infection in children in this area. During treatment
antibacterial drugs should be selected appropriately according to the species of the pathogenic bacteria and drug sensitivity results to reduce the development of drug-resistant strains.
