Effects of tirofiban combined with low molecular weight heparin on serum inflammatory factors and vascular endothelial function in patients with unstable angina pectoris caused by coronary artery disease
Objective To investigate the effects of tirofiban combined with low molecular weight heparin on serum inflammatory factors and vascular endothelial function in patients with unstable angina pectoris caused by coro-nary artery disease. Methods One hundred and three patients with unstable angina pectoris caused by coronary artery disease admitted to a hospital in Zhengzhou from May 2018 to June 2019 were selected for the study
and the patients were divided into an observation group(n=52) and a control group(n=51) using a random number table. The patients in the control group were treated with conventional Western medicine + low molecular weight heparin
and the patients in the observation group were treated with conventional Western medicine + tiroban combined with low molecular weight heparin
and both groups were treated for 7 d. The clinical efficacy
and vascular endothelial function indexes [von Willebrand factor(v WF)
nitric oxide(NO)
endothelin(ET)] before and after treatment were compared between the two groups. Results The total effective rate of treatment for patients in the observation group was 92.31%(48/52)
which was higher than that of the control group at 76.47%(39/51)
and the difference was statistically significant(P<0.05). Before treatment
there was no statistically significant difference in the levels of serum IL-6
hs-CRP
TNF-α
v WF
ET and NO between the 2 groups(P>0.05); after treatment
the lev-els of serum IL-6
hs-CRP
TNF-α
v WF and ET in the 2 groups were lower than those before treatment
and those in the observation group were lower than those in the control group
and the difference was statistically sig-nificant(P<0.05); after treatment
the serum NO levels of patients in the two groups were higher than before treatment
and the patients in the observation group were higher than the control group
and the difference was statistically significant(P<0.05). Within 7 d of treatment
1 case of recurrent angina occurred in the observa-tion group and 4 cases of recurrent angina and 2 cases of nonfatal myocardial infarction occurred in the control group; the incidence of the primary endpoint event was 1.92% in the observation group
which was lower than that of 11.76% in the control group(χ2=3.937
P<0.05). Conclusion Tirofiban combined with low molecu-lar weight heparin can not only improve the clinical efficacy of unstable angina caused by coronary artery disease and reduce the incidence of primary endpoint events such as recurrent angina
but also effectively inhibit the vas-cular inflammatory response and improve the vascular endothelial function.
