Clinical efficacy of ureteroscopic pneumatic ballistic lithotripsy plus transurethral resection of prostate for the treatment of benign prostatic hyperplasia complicated with bladder stones
Objective To assess the clinical efficacy of ureteroscopic pneumatic ballistic lithotripsy plus transurethral resection of prostate(TURP)for the treatment of benign prostatic hyperplasia(BPH)complicated with bladder stones. Methods A total of 106 patients with both BPH and bladder stones who were treated at a Henan hospital between October 2015 and November 2018 were included in this study.Based on a random number table
patients were randomized to receive suprapubic cystolithotomy plus TURP(control group
n=53)or ureteroscopic pneumatic ballistic lithotripsy plus TURP(trial group
n=53)
respectively.The time to stone clearance
intraoperative blood loss volume
duration of bladder irrigation
duration of catheter indwelling
length of hospital stay
occurrence of complications
pre-and post-operative International Prostate Symptom Score(IPSS)
and levels of inflammatory factors were compared between the two groups. Results The time to stone clearance was similar between the two groups(P>0.05).The intraoperative blood loss volume in the trial group was significantly smaller than that in the control group(89.37±16.28 mL versus 132.94±20.03 mL
P<0.05).The duration of bladder irrigation
duration of catheter indwelling
and length of hospital stay in the trial group were significantly shorter compared with the control group(27.73±3.24 hversus 56.48±5.37 h
3.76±0.26 dversus8.89±2.88 d
6.21±2.26 dversus 11.44±4.27 d
all P<0.05).The baseline IPSS scores were comparable between the two groups(P>0.05);at 2 months after surgery
the IPSS scores were reduced from baseline in both groups
with a significantly greater reduction in the trial group(P<0.05).The baseline serum levels of IL-10
IL-6
CRP and TNF-αwere similar between the two groups(all P>0.05);at 7 days after surgery
all of these levels were reduced from baseline in both groups
with significantly greater reductions in the trial group(all P<0.05).The incidence of complications was similar between the two groups(P>0.05).Conclusion For patients with BPH and bladder stones
ureteroscopic pneumatic ballistic lithotripsy plus TURP is reliably safe and effective
which may not only relieve prostate symptoms and enhance quality of life
but also improve clinical biomarkers and reduce complication risk.
