Objective To analyze the clinical therapeutic effect and adverse reaction of insulin glargine in combination with repaglinide on type 2 diabetes mellitus and chronic renal insufficiency.Method 80 patients with type 2 diabetes mellitus and chronic renal insufficiency treated in our hospital from 2010 to 2011 were divided into experimental group and control group randomly with 40 cases in each group.The control group was treated with repaglinide while the experimental group was treated with insulin glargine and repaglinide.Indexes such as fasting plasma glucose（FPG）

2 h postprandial plasma glucose（2h PG）

glycosylated hemoglobin（HbA1c）

c-peptide（C-P）

blood urea nitrogen（BUN）

serum creatinine（SCr）

24 h urine albumin excretion（24h UAE） and the adverse reaction were analyzed after 12 weeks of treatment.Results FPG

SCr and 24h UAE in experimental group and control group after the treatment were reduced compared with those before the treatment

but reduction in the experimental group was more prominent than that in the control group

and differences between the two groups were statistically significant（p<0.05）;the C-P level in experimental group and control group after the treatment were increased compared with that before the treatment;C-P level in the experimental group was higher than that in the control group

and the differences were statistically significant（p<0.05）.Incidence of adverse reaction in the two groups was 17.5% and 20% respectively

and the differences had no statistical significance（p>0.05）.Conclusion The therapeutic effect of insulin glargine in combination with repaglinide in treating the type 2 diabetes mellitus and chronic renal insufficiency was better than that of the repaglinide

which can effectively control the blood glucose change in patients and meanwhile protect the kidney well.
