MR spectroscopic changes of cerebellum in vascular dementia patients
Objective To investigate the metabolic changes of cerebellum in the patients with vascular dementia (VD) by 1Hmagnetic resonance spectroscopy (1H-MRS)
and the relationship between the changes and cognitive impairment in them. Methods Montreal cognitive assessment (MoCA) and Minimental State Examination (MMSE) were used to evaluate 40 stroke patients admitted in our hospital from October 2013 to December 2015. According to the results of cognitive ability after stroke
the patients were divided into VD and non-VD groups. T1WI
T2WI
DWI and 1HMRS examination were performed on the 2 groups of patients and 20 healthy controls. The ratios of N-acetyl aspartate to creatine (NAA/Cr) and choline complex to creatine (Cho/Cr) in the cerebellar hemisphere and cortex were calculated. Moreover
the correlations between the score of MoCA and the ratios of NAA/Cr and Cho/Cr were analyzed in all post-stroke patients. Results ①The ratios of NAA/Cr and Cho/Cr were statistically different between the ipsilateral and contralateral cerebellar hemisphere in the VD patients (P0.05). ②The ratio of NAA/Cr was decreased (P0.05)
while that of Cho/Cr was increased (P0.05) in the ipsilateral cerebellar hemisphere of the non-VD patients than the left cerebellar hemisphere of the healthy control. ③The ratios of NAA/Cr and Cho/Cr in the contralateral cerebellar hemisphere in the DV patients were both correlated with the score of MoCA (r=0.590
P=0.004; r=0.569
P=0.006). Conclusion The ratios of NAA/Cr and Cho/Cr have changed in the contralateral cerebellar hemisphere in the DV patients
which indicates that metabolic disorder may participate in the occurrence of VD.
