Clinical analysis of residual cervical resection after subtotal hysterectomy in 27 cases
Objective To study the clinical outcomes of residual cervical resection after subtotal hysterectomy in 27 cases for providing better strategy in treatment and preventing postoperative complications. Methods The clinical data of 27 cases of residual cervical resection after subtotal hysterectomy in Southwest Hospital from January 2012 to December 2014 was retrospectively reviewed. The subtotal hysterectomy was performed in other hospitals and residual cervical resection was operated in our hospital. Among them
2 cases underwent open surgery
24 cases of laparoscopy
and 1 case converting from laparoscopic to open surgery. Results The age of patients undergoing subtotal hysterectomy was from 29 to 52 (averagely 44.2±6.7). Two cases were due to postpartum hemorrhage
other 25 cases was based on patients’ intention to retain the cervix. The average interval of residual cervical resection after subtotal hysterectomy was 53 months (0.5~251.0 months). There were 8 cases of cervical stump myoma
and 19 cases of cervical stump carcinoma. 4 cases occurred postoperative complications
including 1 case intestinal fistula
1 case intestinal obstruction
1 case abdominal abscess accompanied with pneumonia
and 1 case vesicovaginal fistula. Compared with 482 cases who underwent total hysterectomy during the same period in our hospital
residual hysterectomy had longer surgical time
more intraoperative blood loss
increased period of hospital stay
and the cost was more expensive. The difference between the two surgical procedures was statistically significant (P0.05,P0.01). Conclusion The residual cervical resection should be performed by experienced surgeons with optimal surgical approach e.g. laparoscopy or laparoscopyassisted abdominal/vaginal hysterectomy. In case of subtotal hysterectomy strongly required by patients
preoperative cervical and endometrial malignant lesions should be excluded
and routine postoperative follow-up of cervix is needed.
