Wrist arthroscopy combined with ulnar shortening osteotomy for ulnar impaction syndrome
Objective To explore the treatment efficiency of wrist arthroscopy combined with ulnar shortening osteotomy for the ulnar impaction syndrome. Methods From May 2009 to May 2013
13 cases of ulnar impaction syndrome were treated by wrist arthroscopic surgery combined with ulnar shortening osteotomy. Among these patients
9 were men and 4 were women
aged from 22 to 63 years old (average of 46.2 years old). Xray
visual analogue scale/score and Cooney wrist function score were adopted to assess the preoperative and postoperative wrist function and evaluate the clinical effect. Results All patients achieved bone healing of ulnar osteotomy
without infection or nonunion. The ulnar variance measured by Xray was 3.1±1.5 mm before operation
and -0.3±0.6 mm after operation. The pain of all patients was alleviated at different levels
with the preoperative visual analogue scale(VAS) score of 7.6±1.2 to postoperative score of 3.1±1.6. The pain of 3 patients got complete remission
7 got obvious relief
and the rest 3 obtained less obvious relief. The preoperative Cooney wrist function score was 61.2±16.3
which increased to 86.5±9.8 after operation. According to their outcomes
13 patients were divided into 4 grades
including 3 in top grade
7 in good grade
3 in general grade and no one in bad grade. All data were analyzed with SPSS 13.0
and the preoperative and postoperative scores had significant differences (P0.05). Conclusion Wrist arthroscopy operation combined with ulnar shortening osteotomy is the first option to treat ulnar impaction syndrome
and this method can overcome the shortcomings of single operation and keep the advantages of less trauma
